The Mass General Brigham Medical Group is focused on eliminating barriers to care. Guided by research and community needs, the organization is launching a new Medicaid-focused wrap around clinic. This program is designed to increase access to affordable, high-quality care for approximately 1,600 Medicaid Accountable Care Organization (ACO) patients who may require home-based provider services. To support the launch of this program, the organization is hiring four Advanced Practice Providers, as well as additional clinical and administrative team members, to deliver care within the community at patient homes and other settings. We anticipate these APPs will see patients in our Northern Region with our most urgent needs currently in the Haverhill/Lawrence area, the Lynn/Salem area, and the Saugus/Medford/Revere/Chelsea area. These APPs will be the primary care providers (PCPs) of record and instrumental in building strong and lasting relationships with patients by delivering full spectrum primary care services, providing continuity of care, managing complex health conditions and offering education to help patients understand and participate in their care to achieve improved outcomes. The APPs will collaborate with a physician team member. Additionally, the APP will have the opportunity to work with various interdisciplinary providers, such as community health workers, social workers, nurses, and medical assistants who will extend the reach and effectuate the plan of care by conducting coordinated community outreach. The Medical Group is seeking an experienced, flexible, full-time, 40-hour Advanced Practice Provider whose weekly schedule will be comprised of home-based and community-based care, complemented by some in-clinic care. Each provider will be assigned a panel of approximately 400 patients. The NP is responsible for the assessment and management of various populations of patients, medical and/or surgical, including diagnostic and therapeutic interventions, development of appropriate plans of care and ongoing evaluation. Provides light housekeeping and maintains the cleanliness and neatness of resident apartments. Completes residents' laundry. Assists with meals including light meal preparation or warming. MINIMUM REQUIREMENTS Education: High School Diploma or GED. Nursing assistant or home health certification. Experience: One year of experience. Other Credentials: AHA Heartsaver AED, Drivers License Knowledge and Skills: Possesses a friendly demeanor. Excellent customer service skills. Medical experience preferred. Special Training: Experience with direct and indirect patient care. Mental, Behavioral and Emotional Abilities: Develops strong interpersonal relationships with coworkers and resident base. Usual Work Day:8 Hours Reporting Relationships Does this position formally supervise employees? ESSENTIAL FUNCTIONS Maintains a working relationship with the CMA, LPN, RN, DON, Assisted Living administrator and all other nursing staff, volunteers, Capital Health PACE, administrative staff, hospital employees, department heads and supervisors, outside vendors and contractors, caregivers, family members, and property and building staff. Maintains a professional relationship with all residents. Maintains confidentiality of verbal and written information pertaining to residents, ALP operations and other personnel. Maintains residents' privacy and dignity. Encourages residents to their independence. Possesses a full understanding of fire, emergency and safety procedures. Adheres to policies, procedures, and standards regarding infection control. Understands the hazards and safety of oxygen being used by residents. Maintains frequent close communication with the medication assistant, LPN, and RN. Provides morning and evening personal care to assigned residents including partial or complete bath, oral hygiene, care of hair, incontinence care including changing adult briefs as needed. Provides ostomy care. Takes weights and vital signs including temperature, pulse, respiration and blood pressure monthly and as requested by nurse, records, and reports results. Provides ambulation assistance as needed. Assists with transfer to and from bed. Reports any change in resident status to nurse immediately. Responds to emergency calls and assists nurse as needed. Reports any accidents, injuries, or complaints to nurse or administration immediately. Delivers supplies and equipment to residents. Provides light housekeeping. Cleans refrigerator, stove or oven, cabinets, and silverware drawers as needed. Vacuums and dusts as needed. Assists with laundry by washing, drying, and folding laundry and putting it away. Strips and remakes bed as needed at least once a week. Cleans the bathroom including bathroom floor, vanity, toilet, and shower or bathtub. Removes trash from resident apartments daily. Assists resident with meal service as needed including meal preparation and cutting up foods, set-up, clearing and washing dishes. Reports any unsafe equipment to nurse or administrator. Maintains the cleanliness and neatness of the wellness office, laundry room, equipment, and inventory supplies. Removes trash from wellness offices and laundry rooms. Abides by established guidelines of the facility and governing regulations. Performs other duties as assigned.
