The Cedars-Sinai mentor program, COACH (Community Outreach Assistance for Children’s Health) for Kids, is a mobile medical clinic. It centers on addressing the immediate medical, behavioral health, and social needs of underserved children in South Los Angeles. With a community-centered strategy, the program provides free, temporary care and connects families to continuous, quality healthcare and essential community resources.
Mentor for Kids provides services directly within the community via a brand new, fully equipped, wheelchair-accessible mobile medical clinic. The schedule typically covers Monday through Thursday from 9:00 a.m. to 3:00 p.m., with occasional weekend engagements. Care takes place at accessible community venues such as schools, places of love, public housing developments, grocery stores, WIC (Women, Infants, and Children) centers, Head Start programs, homeless shelters, and other neighborhood-based organizations throughout Central and South Los Angeles, including Watts and Crenshaw.
A core goal of the program is to ensure continuity of care. It connects patients to Federally Qualified Health Centers (FQHCs) and other medical homes. These providers support the long-term health needs of the entire family.
COACH for Kids reduces barriers to care and improves health outcomes by meeting families where they are and supporting their transition into comprehensive, continuous healthcare systems.
Responsibilities:- Provide comprehensive primary care to pediatric patients in community settings via a mobile clinic, including well-child visits and preventive services.
- Conduct thorough histories, physical exams, and assessments; develop individualized care plans.
- Diagnose/manage acute and chronic conditions; perform diagnostic and therapeutic procedures.
- Administer immunizations, screenings, and point-of-care testing; interpret results and follow up.
- Educate patients and families on health promotion and disease prevention, tailored to their needs.
- Collaborate with healthcare team and community partners for coordinated, high-quality care.
- Facilitate referrals and coordinate with FQHCs, specialty care, and community resources.
- Serve as onsite clinical lead, supervising staff and ensuring efficient mobile clinic operations.
- Document patient care accurately in electronic medical records.
- Support and mentor clinical staff and students; contribute to quality improvement initiatives.
- Participate in community outreach and health education events.
- Maintain clinical knowledge through continuing education and uphold Cedars-Sinai standards.
- Ensure a safe and inclusive care environment for underserved and diverse populations.
- Operate in field settings, including mobile and pop-up clinics.
Qualifications
Education:
- Master of Nursing Degree.
- Graduate of an accredited Nurse Practitioner program approved by the California Board of Registered Nursing.
Experience:
- At least one year of experience working as a Nurse Practitioner in pediatric clinical environments is required.
- Experience working in community-based settings, particularly with underserved populations.
Licensure/Certifications Requirements:
- CA RN License.
- Nurse Practitioner and Furnishing License.
- National Certification as a Nurse Practitioner credential.
- DEA License (schedule 2-5).
- Current Life Support Certification - ACLS and PALS required.
