JOB SUMMARY The certified nursing assistant is an integral member of the health care team who delivers quality care through collaboration and communication with team members. They work under the direct supervision of the registered nurse to support and assist the nurse in the delivery of quality care to patients and families. ESSENTIAL FUNCTIONS PATIENT / CUSTOMER Essential Accountabilities Report Receives written/verbal report at the beginning of each shift from the registered nurse (RN) regarding the care that is to be delivered to the assigned group of patients, or for patient needing constant observation. Gives report when leaving unit, and at the completion of the shift regarding the care that was delivered; any abnormalities that occur during the shift are to be reported as they occur to the RN as well as to the oncoming CNA. Assists in Assessment Identifies the patient per hospital policy before carrying out functions. Assists with data collection for nursing history and physical e.g. vital signs, ht. wt, blood sugar results, functional status and communicates changes to the RN in a timely fashion. Participates in respectful communication with patients, families and coworkers. Provides patient and family information to the registered nurse for coordination of care and discharge planning. Observes skin changes and reports discoloration, abrasions, infestations or skin breakdown. Planning Assists in setting mutually agreed upon realistic individual patient goals. Integrates the RN’s directions re: the patient’s plan of care into the care given to the patient. Gives RN report before the end of the shift regarding the patient’s response to the plan of care issues, and at various times during the shift to report unusual findings e.g. abnormal blood sugar results, vital sign changes, pain etc. Intervention Promotes infection control by using the appropriate hand washing techniques. Assists with colostomy irrigations. Assists with vaginal irrigations. Applies urosheath/condom catheter. Removes foley catheter as directed by the nurse and monitors urinary output as directed. Administers cleansing enemas as ordered. Performs post mortem care. Assists the physician to perform physical exams. Assists with oxygen therapy only and does not regulate the flow of oxygen. Encourage the use of the pulmonary toilet by using the incentive spirometer, coughing, turning and deep breathing. Utilizes the appropriate equipment for patient lifting. The In Home Services CNA is responsible for providing routine care and assisting participants or clients with activities of daily living, restorative and supportive care. Physical attendance at an InnovAge facility and/or an InnovAge participant home is essential to performing the job duties. Follows plan of care and assists participants with activities of daily living such as bathing, dressing, grooming, hair care, mouth care, eating, hand and toileting as assigned or needed. Documents care provided in accordance with established policies, guidelines and applicable regulations Carries out, reports and documents care given in an effective, timely manner as observed by the RN and through periodic record reviews. Participates in the care planning of participants through cooperation with the multidisciplinary teams. Monitors participants’ health status by taking vital signs and weights (monthly). Monitors appetite in addition to looking for changes in functional ability and checking skin condition while assisting with toileting. Monitors/assists participants in a courteous manner with meals and provides assistance with feeding as necessary. Assists participants with restorative and supportive care such as ambulating, positioning, transferring, skin care, catheters, fills oxygen and provides other aides as assigned or needed. Monitors participants’ health status such as vital signs, weights, skin condition, appetite and changes in functional ability and reports pertinent information to clinical supervisor in a timely manner. Reports any variance, accident or unusual occurrence to the appropriate personnel. Attends in-service and continuing education programs as scheduled and necessary. Perform nursing assistant services to clients in their place of residence Attends position related in-services. Attends all mandatory in-service programs as scheduled; minimally 12 hours/year Participates in QM activities as requested. Completes appropriate visit records in a timely manner as per Agency policy Performs simple procedures as an extension of the therapy or nursing services as assigned. Performs household services essential to health care in the home as assigned. Does not accept assignments for a patient with special needs for which he/she has not received appropriate training. Additional work functions and duties may be assigned REQUIRED Current and valid state issued Certified Nursing Assistant (CNA) license Current First Aid and BLS certifications are required prior to hire . Acceptable vendors for certifications are from either American Heart Association and/or American Red Cross. Require personal transportation, current state issued driver’s license, good driving record and auto insurance as required by law A minimum of one year experience working the frail or elderly. High School Diploma / GED or relevant work experience in lieu of education.
