The Opportunity The Licensed Caregiver – LPN is part of the Health & Wellness Team responsible for providing high-quality care to our residents under the supervision of registered nurses and healthcare professionals. What You’ll Do Supporting residents with medication. Take vital signs, such as blood pressure, heart rate and pulse Observe and report on residents’ health, escalating any problems efficiently and effectively Provide emotional support and serve as a source of comfort for residents and their loved ones Recognize abnormal changes in body functioning and report to a supervisor Perform nursing assessments, including vital signs monitoring, physical examinations, and health status evaluations, to identify and address resident needs promptly. Provide nursing interventions as directed by the registered nurse or healthcare provider, following applicable state regulations, established protocols and infection control practices. Responsibilities include customer service, safety, resident care, and communication. A Licensed Practical Nurse may delegate nursing activities to assigned health care personnel. The delegating Licensed Practical Nurse shall bearfull responsibility for making an appropriate assignment, adequately teaching, directing and supervising the delegate(s), as well as the outcome of that delegation; all within the parameters of their generic and continuing education and experience. Provides quality care to children and young adults in accordance of the Nurse Practice Act of Massachusetts. Responsibilities •Assess and coordinate healthcare services for Program Manager. Institute appropriate follow up and communicate all medical issues to the program Residential Registered Nurse and any other appropriate Seven Hills Pediatric Center at Hopedale staff members.•Assist individuals in designated programs to their medical appointments and with transportation as recommended by the Program Manager, Registered Nurse, or the Nurse Practitioner.•Document all nursing interventions and changes affecting the children in appropriate areas.•Review the plans of care for current knowledge of programming as required by the Department of Children and Families and/or other agencies.•Maintain current medication and treatment orders noting any side effects and/or adverse effects and report to appropriate individuals: MD, NP, Program Manager or program Residential RN, and other designated Seven Hills Pediatric Center at Hopedale staff. Assist in training staff in medication policies and procedures.•Maintain health records according to various government agencies as required. Ensure that information is communicated to residential staff and school program regarding recommendations and treatment.•Consult with residential Registered Nurse for supervision and direction as needed to ensure appropriate medical health care is provided for participants.•Attend staff meetings to present all current medical/health concerns and any upcoming medical appointments to team.•Complete all mandatory in-service education in a timely manner and attend any educational programs at the recommendation of the Program Manager.•Implement procedures and services including medical interventions, or additional duties as assigned by the Program Manager or residential Registered Nurse.•Ensure that residents are sufficiently observed to maintain their safety and optimal health.
