Job Description The OR RN is responsible for coordinating care delivery within the OR/procedural setting. Must demonstrate strong communication skills, work well in a multi-disciplinary setting and be a strong patient advocate. SPECIFIC EDUCATIONAL/VOCATIONAL REQUIREMENTS: 1 A graduate of an accredited School of Nursing with a current registered nurse licensure by the New Jersey State Board of Nursing. 2 A minimum of three years full-time or equivalent clinical experience is required and a minimum of two years of clinical experience in long-term care nursing with one year in a management/administrative or supervisory capacity is preferred. 3 BSN preferred. The Opportunity The Certified Caregiver - Medication Aide position provides support for our community’s residents with the essential day-to-day tasks they can no longer perform or need help with. Certified Caregivers are responsible for administering daily medications. Duties include monitoring residents, reporting changes, and collecting samples. The ideal Certified Caregiver will be detail oriented, possess knowledge of resident care procedures and have excellent communication skills. What You’ll Do Deliver routine daily medications, either prescription or non-prescription, to residents Record medication dosages and times Observe residents and document changes in their condition Gather samples for analysis Ensure equipment is routinely inspected and cleaned Uphold all health and safety standards Follows federal, state, and agency regulations to protect own health and that of others Coordinate with different nurses to assist with resident care and medications Follow written or verbal instructions on how to manage medications Respond to resident emergency call signals, bells, or intercom systems to identify residents needs Protects confidential client information unless otherwise required to promote client safety and welfare Seeks guidance from the nurse as needed to perform tasks safely and efficient Responsibilities include customer service, administering medication, caregiving, resident safety and communication What You’ll Bring POSITION REQUIREMENTS / QUALIFICATIONS: Successful completion of a state approved Medication Aide course and is approved to assist with or administer oral and external medications, per state regulations Must demonstrate competency to perform medication administration in accordance with state regulations High School Diploma or equivalent preferred, although mandatory in certain states Applicants must be a minimum of 18 years old, or the age required by state regulations, whichever is higher Working experience alone, on a team, and on projects start to finish Strong emotional intelligence and listening skills Dedication to the medical field, especially towards working with the residents Must pass State and company background checks and drug test Take Vital signs Assisting resident with Activities of Daily Living (ADLs) Medication supervision (within the scope of the state regulations) Read, write, speak and understand the English language 
Swan Hospice is looking for CHHA ( Certified Home Heath Aid ) with Hospice experience to join our team. We provide quality hospice care to patients in skilled nursing facilities, assisted living facilities and in private homes. Job Responsibilities Assists with the prescribed exercise and ROM activities which the resident/patient and Hospice aide have been taught by the Hospice Nurse. Provides assistance in toileting; positioning and transfer activities, frequent turning and positioning of a bed bound resident/patient. Provides or assists in ADL activities; personal hygiene, grooming, mouth, skin care and nails. Assists or reminds the patient to take medication, in compliance with regulations of appropriate regulatory and accrediting bodies. Assists with light housekeeping of individual's personal unit or room, including changing of bed linen, if needed and patient laundry. Feeds and assists with meals. Obtains temperature, pulse and respiration as supervised by the Hospice Nurse. Reports any changes in resident's/patient's condition to the Hospice Nurse and, if in a facility, to the facility nurse and documents the information , including the date and time of the report and the person receiving the information. Ideal candidates will have: HHA License in the state of NJ Understands the hospice philosophy Minimum of one year experience working in hospice Dependable transportation
