The Clinical Director of Nursing is responsible for the overall direction, coordination, and management of all clinical hospice services. This role ensures that quality, safe, and compliant care is provided to patients and families in accordance with the philosophy, mission, and policies of Swan Hospice, as well as all federal, state, and accreditation requirements. Essential Duties & Responsibilities Direct, supervise, and evaluate the performance of clinical staff including nurses, aides, social workers, chaplains, and therapists. Ensure compliance with all CMS, state, and accrediting body regulations governing hospice care. Participate in the development and implementation of policies, procedures, and best practices. Oversee patient care delivery, ensuring individualized and compassionate plans of care. Lead Interdisciplinary Team (IDT) meetings, fostering communication and collaboration among staff. Monitor quality improvement initiatives and patient/family satisfaction. Manage staffing assignments to ensure adequate coverage for patient needs across service areas. Provide direct nursing care as needed in complex or urgent situations. Collaborate with the Administrator/CEO on strategic planning, budgeting, and program development. Act as a resource, mentor, and educator for clinical staff. Ensure timely and accurate clinical documentation in compliance with regulatory standards. Participate in hiring, onboarding, training, and disciplinary actions for clinical staff. Serve as the primary clinical contact for physicians, referral sources, and regulatory agencies. Qualifications Current Registered Nurse (RN) license in [state]. Minimum of 3–5 years of clinical nursing experience, with at least 2 years in hospice, palliative care, or home health. Prior leadership/management experience required. Strong knowledge of Medicare/Medicaid hospice regulations. Exceptional organizational, leadership, and communication skills. We provide quality hospice care to patients in skilled nursing facilities, assisted living facilities and in private homes. We treat residents, families and loved ones with kindness, respect, compassion and dignity. We have a comprehensive team that can provide support and stability to residents and staff. Job Responsibilities Assists with the prescribed exercise and ROM activities which the resident/patient and Hospice aide have been taught by the Hospice Nurse. Provides assistance in toileting; positioning and transfer activities, frequent turning and positioning of a bed bound resident/patient. Provides or assists in ADL activities; personal hygiene, grooming, mouth, skin care and nails. Assists or reminds the patient to take medication, in compliance with regulations of appropriate regulatory and accrediting bodies. Assists with light housekeeping of individual's personal unit or room, including changing of bed linen, if needed and patient laundry. Feeds and assists with meals. Obtains temperature, pulse and respiration as supervised by the Hospice Nurse. Reports any changes in resident's/patient's condition to the Hospice Nurse and, if in a facility, to the facility nurse and documents the information , including the date and time of the report and the person receiving the information. Job Description A. BASIC PURPOSE Oversees primary caseload of Hospice patients and provides skilled professional nursing care to them, their families and caregivers as prescribed by the physician and in compliance with the Hospice Medicare Conditions of Participation, New Jersey Hospice licensure laws, accrediting body standards and agency policies and procedures. Provides the initial assessment and comprehensive nursing assessment and identification of problems of each patient; reassessment of nursing needs and collaborates to develop the Hospice Interdisciplinary Team (IDT) plan of care. Implements the plan of care and reevaluates and revises the plan of care relative to the patient’s nursing needs to provide appropriate, comprehensive and responsive goals and interventions. Teaches, supervises, and counsels the patient, family members, and staff regarding nursing care and the patient's needs, including other related problems of the patient at home Works closely with other members of the IDT to ensure all patient and family care needs are being met. Provides supervision to Hospice Aides/Homemakers. 2. Experience a. Has minimum of one (1) year general nursing preferred with current hospice, home health, medical, surgical or critical care experience. b. Experience with pain and symptom management interventions preferred. c. Ability to work within an interdisciplinary setting. d. Understanding of Hospice philosophy, principles of death/dying.
