In this role, the RN provides nursing care to assigned patients in accordance with ANA standards, the New Jersey Practice Act, Joint Commission requirements, and established hospital and Nursing Department policies and procedures. The RN also assists physicians by monitoring patients during cardiac testing procedures to ensure safety and accurate clinical assessment. Responsibilities Obtains a complete nursing history and baseline assessment that is systematic, consistent, and continuous. This assessment is communicated effectively and documented. Appropriately analyzes health status data to determine nursing diagnosis and needs for appropriate nursing care, medical intervention or services of other clinical professionals. Identifies patient problems and formulates nursing diagnosis according to physical, psychological, spiritual/cultural and educational needs in conjunction with a multi disciplinary approach. Consistently includes patient and family in the development of an individualized plan of care. Administers medications as needed following CSHS policies and manufacturer's instructions. Takes immediate and appropriate actions in the event of a medication variance occurrence. Maintains and upgrades own nursing practice and development. Recognizes adverse reactions to medications and takes immediate and appropriate actions. Recognizes changes in patient condition and/or emergencies situations and takes immediate and appropriate actions. Participates in unit conferences to discuss quality issues and quality improvement strategies. Delegates components of care to non-professional caregivers as appropriate. Makes decisions that demonstrated knowledge of nursing practice; utilizes sound nursing judgment in clinical and patient care management situations. Demonstrates critical thinking skills in the care of patients and in the coordination of care to assure the achievement of maximum outcome Provides appropriate interventions for pain. Documents pain assessment, intervention and reassessment. Notifies physician for unrelieved pain as indicated. Provides patient/family education regarding cardiac risk factors. Introduces plan for healthy lifestyle changes. Educates patient in cardiac disease process and introduces cardiac rehab recovery plan. Qualifications Bachelor's degree in Nursing preferred. Two years experience in telemetry or critical care required. Experience in emergency care considered. Current NJ RN License required. CPR Certification required. ACLS Certification required. The RN Supervisor will assess new and incoming home care clients, as well as supervise and educate field staff on clinical items and care needs, and act as a liaison between the office and clients. This position will be responsible for both in-person and telephone nursing assessments and field staff supervisions primarily within Middlesex and Somerset Counties, with occasional supervisions within Passaic and Bergen Counties. Preferred candidate will be comfortable with traveling, based on business needs. The schedule for this position is Monday and Friday day time hours and availability every other weekend for new client visits. The position is primarily in-person with occasional opportunities to work remotely depending on the needs of our clients. Responsibilities: Perform Initial Assessments, Reassessments, Case Monitoring, and other assessments for home care clients, consistent with standards of nursing practice and CAHC guidelines. Develop and implement the written care plan that reflects the needs of the client. Complete clinical competency and administrative supervision of field employees, including CHHAs and companions. Assist in providing quality care that meets or exceeds company and regulatory standards. Assess and document the client’s biophysical, psychosocial and educational needs as well as accurately evaluating and documenting changes in client’s status. Demonstrate the ability to communicate effectively with the client and his/her family members, as well as the health care team/staff of the agency. Report appropriate changes in the client’s condition to the Director of Nursing and/or the physician. Consistently adheres to universal precautions, aseptic technique and infection control guidelines. Implements care in a manner that is maximally safe for the client, his/her family and self. Maintains standards of nursing care as well as implement the policies and procedures established by the agency. Respect the rights, privacy and property of all others at all times. Requirements: At least 3 years of previous RN experience. Previous RN supervisor or home care experience is preferred. Active New Jersey RN License in good-standing. Willing to travel throughout New Jersey; expected travel can be 60-70 % of scheduled job responsibilities. Basic knowledge of Microsoft Office Software and/or ability to utilize online software to complete charting and required paperwork. Up-to-date medical documents, including recent PPD or chest x-ray.
