Documents all nursing care, change in condition, response to therapy, treatments, procedures and referrals on patient record to meet standards. Demonstrates dependability, reliability and flexibility. Stewardship: Uses and cares for equipment and supplies economically. Customer Experience: Provides patient, family, and/or caregiver education as directed by the plan of care. Provides service to patients and families with sensitivity and respect for their needs, expectations, age, cultural, and individual differences. Contributes to the patient experience as evidenced by positive patient satisfaction scores and HCAHPs scores for department/medical center and/or written compliments by patients/others as well as consistent performance of purposeful hourly rounding and bedside shift report. PRIMARY ACCOUNTABILITIES Evaluate and educate associates regarding their illnesses, injuries, and exposures. Perform annual Associate Wellness evaluations and update immunizations. Ensure associate compliance of N-95 and influenza requirement. Job Requirements POSITION SUMMARY The Certified Medical Assistant provides and ensures patients receive whole care in all aspects of an office visit, regarding intake, test results, screenings, labs, medication administration, closure of care gaps while delivering excellent customer service to the patients, families and other care Providers, focusing on patient-centered care. PRIMARY ACCOUNTABILITES Accurately prepares and performs point of care testing such as urine dipstick, blood glucose, etc. Performs clinical intake to include vital signs, medical history, surgical history, social history, and verification of patient’s current medications. Prepares and administers medications as ordered by provider utilizing the “6 Rights” of Medication Administration, to include documentation and coding. Controlled substances, insulin and allergy doses must be checked by a Licensed Practical Nurse (LPN), Registered Nurse (RN) or Licensed Practitioner. Assists in stocking and orders supplies as well as maintains equipment and coordinates medical supply ordering with site manager. The RN Case Manager uses sound clinical judgment, working collaboratively with the interdisciplinary team, patient, family, and significant support personnel to achieve optimal clinical outcomes through effective utilization of resources and discharge planning for all hospital admissions. PRIMARY ACCOUNTABILITIES Initiates care transition at time of admission, developing and revising individualized discharge plans (as indicated by assessment/reassessment and response to treatment) to meet patient needs during their hospital stay, manage their length of stay, promote efficient utilization of resources, and plan for a safe discharge. Identifies at-risk populations using approved screening tool and following established referral processes for safe transition to next level care to prevent readmission. Practices autonomously, consistent with evidence-based standards. Serves as a case management resource expert for compliance with Medicare IM letter, 3008, 1823 PASSR, and DCF mandatory legal reporting, etc. Works with the Utilization Department, Physician Advisors, Hospitalists, Emergency and other physicians to ensure appropriate, cost-effective care with the best patient outcomes. Proactively identifies and either resolves or escalates delays and obstacles to discharge. Identifies reimbursement factors influencing choice of post-hospital providers and obtains authorizations as required, either directly or in collaboration with the payer’s case manager. Evaluates overall patient daily plan for effectiveness and involves patient and family in formulating plan goals. Works collaboratively and maintains active communication with physicians, nursing, physician advisor, and other members of the interdisciplinary care team to effect timely, appropriate patient resource management and patient transition.
