POSITION SUMMARY
The Health First Home Care (HFHC) Assistant Nurse Manager manages the Clinical Supervisor team
and supports oversight to include participation in the development and implementation of policies and
procedures for the Home Care department, scheduling and assignment of personnel, and continuous
quality improvement. The HFHC Assistant Nurse Manager serves as clinical and professional role
model to staff and contributing to their professional growth and development as well as fostering
connections with patients both internal and external.
PRIMARY ACCOUNTABILITES
1. Promotes associate accountability and competency through leadership and management
practice. Encourages associate growth and development through mentorship and goal setting.
2. Assists in providing consistent on-boarding experience for all new clinical staff.
3. Provides input for annual performance appraisals and competency checklists for personnel on
an annual basis and as necessary in a timely manner.
4. Interviews and participates in the selection of new clinicians.
5. Assists with implementation of utilization-based performance improvement activities resulting in
improved operations and patient care outcomes.
6. Identifies opportunities for quality improvement activities in the department.
7. Works with individual nursing supervisors and their clinical teams on opportunities to increase
Patient Engagement scores, along with HHVBP incentives.
8. Ensures compliance with the Joint Commission and other organizational and regulatory agency
standards with quality indicators and core measures with on-going education.
9. Monitors and ensures compliance with staffing guidelines, utilization and productivity to meet
financial goals.
10. Communicates staff, equipment needs and concerns to Manager/Director as appropriate.

Work Experience
MINIMUM QUALIFICATIONS
• Education: Any one of the following:
o Bachelor’s degree in Nursing (BSN) OR
o Complete BSN within three (3) years of start date.
• Work Experience:
o Three (3) years Home Health nursing experience and acute care experience as a RN.
• Licensure:
o Registered Nurse (RN) Licensure in the State of Florida or endorsement.
o Valid Driver’s License and clean Motor Vehicle Record and maintained.
• Certification: American Heart Association Basic Life Support (AHA BLS) Healthcare Provider
Completion Card prior to start date and maintained.
• Background Check: Satisfy Florida of Agency for Health Care Administration (AHCA) Level 2
background screening.
• Skills/Knowledge/Abilities:
o Oasis documentation.
o CMS Conditions of Participation.
o ACHA Home Care Standards.
o TJC Standards.
PREFERRED QUALIFICATIONS
• Work Experience: Prior Home Health leadership experience.
PHYSICAL REQUIREMENTS
• Majority of time involves sitting or standing; occasional walking, bending, and stooping.
• Long periods of computer time or at workstation.
• Lightwork that may include lifting or moving objects up to 20 pounds with or without assistance.
• May be exposed to inside environments with varied temperatures, air quality, lighting and/or low
to moderate noise.
• Communicating with others to exchange information.
• Visual Acuity and hand-eye coordination to preform tasks.
• Workspace may vary from open to confined.
• May require travel to various facilities within beyond county perimeter may require use to
personal vehicle.
