Clinic RN

The Clinic RN (Registered Nurse) provides skilled nursing care in accordance with InnovAge policies and all applicable regulations to participants. Assists in the examination, testing, and treatment of participants. Implements orders written by primary care providers. Triages participant and staff phone calls to determine the priority of care and routes accordingly. Schedules outside appointments for participants when necessary. Administers prescribed medications and treatments. Provides training to participants in self-administration of applicable medications and monitors medication compliance. Documents all necessary information and maintains participant medical record(s). Provides health counseling and education to participants and their families. Observes, records, and reports participant’s condition and reaction to drugs and treatments to providers. Assist with clinical teaching and clinical supervision of CNA Day Center staff. Develops, implements, and maintains a current nursing care plan. Supervises LPN/LVN’s and Medical Assistants in addition to evaluating the overall effectiveness of the clinic in the absence of the Nursing Services Director/Clinic Nurse Supervisor. Ensure that exam rooms and/or triage area is stocked for necessary supplies and materials and that appropriate staff maintains a list of medical supplies. Effectively communicates and actively engages in interdisciplinary team meetings, family meetings, and clinic meetings. Participates within the interdisciplinary team in the formulation of Plans of Care participants, as well as in other interdisciplinary team settings. May be required to provide after hours and holiday coverage. Complies with all regulatory and policy and procedure guidelines. RN Wound Care Specialist

The RN Wound Care Specialist supports clinical and nursing home staff in the management of InnovAge admitted participants for acute/chronic wound care and education. They collaborate with the interdisciplinary team daily to coordinate appropriate plan of care as it relates to wound healing. Physical attendance at an InnovAge facility and/or an InnovAge participant home is essential to performing the job duties. Participant Nursing – 70% Assesses, develops, plans, and evaluates care provided to participants while admitted in nursing home settings. Collaborates with physicians, other members of the interdisciplinary health care team, and patient/family in the development, implementation, and documentation of appropriate, individualized plans of care to ensure continuity, quality, and appropriate resources as it relates to wound care. Recommends alternative levels of care and ensures compliance with federal, state, and local requirements. Assesses high risk patients in need of wound care. High risk participants with the diagnosis of peripheral vascular, neuropathy and diabetic. Collaborate with clinical and nursing home staff to develop and coordinate the implementation of a wound care evaluation and treatment plan to meet participants identified needs. Communicates the plan to physicians, participant, family/caregivers, and appropriate nursing staff. Reviews, monitors, evaluates, and coordinates the patient's wound care plan to assure all appropriate and essential services are implemented timely and efficiently. Collaborate with the InnovAge wound clinic to triage participants for priority of care and routes accordingly. If required, visit participant in the nursing home setting to help coordinate care and discharge plan. Provides participants, clinical and nursing home staff with education to assist with healing and preventive measures for wound reoccurrence. Assists in the examination, creation of wound care plan. Processes orders and disseminates orders and instructions to appropriate personnel, in addition to implementing orders written by primary care physician(s) and wound care team. Oversees scheduling of specialist appointments pertaining to wound care Administration – 30% Builds collaborative relationships with external and internal team members. Develops, implements, and maintains a current nursing care plan in cooperation with the Clinic Nurse Manager. Coordinates transfer of patients to appropriate facilities; maintains, and provides required documentation. Maintains and reviews participant records, charts, and other pertinent information. Effectively communicates in interdisciplinary team meetings, family meetings, partnership meetings and clinic meetings. The In Home Services CNA is responsible for providing routine care and assisting participants or clients with activities of daily living, restorative and supportive care. Physical attendance at an InnovAge facility and/or an InnovAge participant home is essential to performing the job duties. Follows plan of care and assists participants with activities of daily living such as bathing, dressing, grooming, hair care, mouth care, eating, hand and toileting as assigned or needed. Provides health counseling to participants and their families on an individual or group basis. Assists in the examination, assessment, evaluation testing and treatment of participants. Observes, records, and reports participant’s condition and reaction to drugs and treatments to physicians Perform nursing care to clients in their place of residence. Assess and re-assess participants to determine individual needs and to ensure the least restrictive environment. Assesses participants to determine change in condition that necessitates a change in level of care and coordinates the plan of care with Home Care resources. Furnishes those services requiring substantial specialized nursing skill. Initiates appropriate preventive and rehabilitative nursing procedures. Prepares clinical and progress notes for each client visit and summaries of care conferences for clients in a timely manner as per Agency policy. Conducts client care conferences on clients assigned to his/her care. Participates in peer review and quality management as assigned. Participates in utilization review of medical records as assigned. Processes orders and notifies physicians of client needs and changes in condition. Completes certification/recertification orders and discharge summaries. Determines the amount and type of nursing needed by each individual client. Ensure that quality care is delivered by home care team to site participants via periodic participant/caregiver inquiry, intermittent review of care plan, and trending of data from various reports/supervisory visits in collaboration with the Home Care Manager. Refers to Physical Therapist, Speech Language Pathologist, Occupational Therapist and Medical Social Worker for those clients requiring their specialized skills. Supervises the LPN every 30-days and supervises the HHA/CNA every 14-days (or per regulatory requirements) and teaches other nursing personnel.
