Communication: Provide referral sources with information on hospice programs and the coordination of patient care for optimal transition from the hospital to discharge destination in a hospice care setting wherever the patient calls home. Operations: Attend interdisciplinary team meetings for full team discussions, consultation and evaluation of patient/family needs. HARD & SOFT SKILLS: Compassionate communicator with a positive attitude. Patience is a virtue when working with patients, families, physicians, and coworkers.
