Anchor Hospital is seeking seasoned Registered Nurse, for Intake Assessment and Referrals. This RN is responsible for delivering quality nursing care to both psychiatric and chemically dependent adult, geriatric and adolescent patients. To assess the needs of walk-in and phone callers and ensure their referral to the service or resource to best address their needs. Other responsibilities include being able to perform all aspects of the hospital’s assessment and referral process including efficient communication with referral sources, immediate patient/client disposition, arranging hospital transfer/discharge, and/or making connections to other community agencies and resources.
