The Licensed Practical Nurse (LPN) works under the direction of the Team Leader. He/she gathers assessment data, monitors, follows plan of care and promotes optimal level of health. As directed by the team leader and the Registered Nurse and in partnership with the health care team and patient/ family, performs work processes to accomplish positive patient outcomes, and organizational goals. Provision of care focuses on psychosocial support and activities of daily living. We do 12 hours shifts from 6:30am-7pm and 6:30pm-7am. We are currently hiring for both shifts, Full and Part Time.
