The Licensed Practical Nurse (LPN) works under the direction of the Team Leader. He/she gathers assessment data, monitors, follows plan of care and promotes optimal level of health. The LPN provides medication administration, treatments, supervision of CENAs performing ADLs. Collaborates with the Care Coordinators and other team members to ensure communication and timely updates on residents change in conditions. We do 12 hours shifts from 6:30am-7pm and 6:30pm-7am. We are currently hiring for both shifts, Full and Part Time. Responsibilities KEY RESPONSIBILITIES INCLUDE: • Gathers assessment data, monitors comprehensive physiological, psychological, sociocultural, and spiritual needs of patients/significant others under the supervision of the Team Leader. Reports findings to the Team Leader. • Provides competent care consistent with the nursing process and written plan of care, physician’s orders and in accordance with skilled nursing facility policies and procedures. • Participates in admission and discharge planning, in collaboration with health care team, at the time of admission and continues through discharge. • Provides patient and family education to meet their specific needs as outlined in the plan of care and delegated by the Team Leader. • Documents care consistent with guidelines. • Performs other duties consistent with purpose of job as directed.

Assists residents to Activities. • Provides/assists with care to support physical function for activities of daily living, such as toileting, bathing, feeding, and ambulation which allow residents to achieve their highest practical level of function. • Documents according to facility guidelines to accurately reflect care provided immediately after performing procedure or task.

The RN uses clinical judgment skills to diagnose and treat patient responses to actual or potential health conditions. Assesses monitors, detects, plans and takes appropriate action to prevent potential physiological complications and promotes optimal level of health. Provides medication administration, treatments, and supervision of CENAs performing ADLs. Accountable for integrating care within scope of practice licensure, relevant education and experience. Collaborates with the Care Coordinators, Team Leaders and other team members to ensure communication and timely updates on resident’s change in conditions. Utilizes Policies and Procedures to guide job duties. We do 12 hours shifts from 6:30am-7pm and 6:30pm-7am. We are currently hiring for both shifts, Full and Part Time.
