We have many schedules available for this role, including weekends and nights. The pay range for this position is $34.00 - $36.00 per hour, depending on experience . This position also qualifies for $1.50 swing shift differential and $3 graveyard shift differential. The schedule for this position is 7:00am to 3:30pm. Workdays may vary. Responsibilities Client Care: Perform initial intake to ensure that an appropriate care plan is established based on discharge information from the hospital or primary care notes Assess health status of patient upon arrival by conducting head to toe assessment and checking vitals Determine the patient’s insurance status, make follow up appointments with patient’s providers and reviewing medication list Establish and maintain a care plan Set weekly goals for patient to complete to assist with medical or social reintegration, verify goal completion, and assess medical needs. Conduct morning, afternoon, and evening checks for all assigned clients to observe wellbeing, ensuring there is no change in medical conditions since hospital discharge.
