Job Summary The Technical Partner provides direct and indirect patient care under the direction of the RN or LPN. Essential Job Functions Deliver direct and indirect patient care in accordance with hospital policies, procedures, protocols and standards of care. Perform and/or assist with patient care needs as directed and assists patient with activities of daily living. Respond to call lights, inquiries and requests from patients and families in a timely manner. Collect, document and report ongoing patient care data as directed. Measure and/or document intake and output, including drainage and obtains daily weights when indicated. Obtain individual patient care equipment as directed. Transport patients as directed. May perform the following skills: obtain vital signs, apply heating pads, set up suction equipment, utilize E cylinders, apply antiembolic hose, aseptic techniques, distribute and maintain sufficient supply of linens, apply Sequential Compression Devices, perform blood glucose testing, perform ostomy care, simple dressing changes, apply telemetry leads, bladder scanning, preventative skin care. Apply pulse oximetry, urinary catheter care, transport blood and blood products from blood bank, perform EKGs, oral pharyngeal suctioning, place patient in CPM machine, prepare and maintain isolation boxes and supplies, declutter patient rooms and storage areas, deliver specimens to lab, crutch measurement and teaching, maintenance of specialty carts, supply inventory and special order supplies, stock warmers with blankets, saline, and water. Make regular rounds to address patient needs and physical environment - purposeful hourly rounding, Assist in the educational process to promote and maintain health, foster self-care, and improve outcomes. Obtain resources as needed. Report to the RN/LPN information regarding the patient's readiness to learn. Maintain a collaborative relationship with other health care providers to achieve desired outcomes and assure continuous flow of patient care. Communicate pertinent information of actual/potential problems to the professional/practical nurse. Communicate pertinent information at shift reports and during internal and external transfers. Participate in the unit performance improvement activities designed to enhance the quality of patient care and customer services. Assist in data collection process as requested. Recommend modifications for performance improvement. Demonstrate leadership by utilizing the behaviors inherent to the role of the technical partner. Demonstrate an ability to solve problems utilizing critical thinking skills and seeks assistance from immediate supervisor when appropriate. Maintain accountability for actions taken. Effectively function as a resource person. Act as a patient advocate. Perform effectively in the role of preceptor/mentor to new hires and/or students. Assist in maintaining/decreasing organizational/departmental costs. Assist in the department's compliance with state, Joint Commission, OSHA and other regulatory agencies. Assist in continuous survey readiness for inspections and surveys. Adhere to the infection control process, including standard precautions, to reduce the risk of acquiring or transmitting infections. Assist with maintaining sufficient and proper supplies on an assigned unit. Assist with monitoring sterile supplies for expiration date and replaces as needed. Provide for patient safety in compliance with hospital and nursing department policies. Utilize two forms of patient identification before implementing any form of patient care. Assume appropriate code team role during code situations or mock codes. Respond effectively during emergency situations, including internal and external disasters. Assist in maintaining a safe and clean environment for patients, visitors and staff. Complete proper collection, identification, labeling and delivery of specimens (including diagnostic studies) to the appropriate department. Support limited use of restraints in accordance with established standards and documents according to policies. Other duties as assigned. Required Qualifications High School Diploma, GED or higher level of education. Sound judgment, problem solving skills, strong oral and written communication skills, ability to follow directions from other team members; works independently assigned duties. Strong ability to communicate complex and/or controversial topics and concepts to a wide and diverse audience. Preferred Qualifications Completion of a Nurse Aide or comparable program (such as EMT, Medical Assistant, nursing student with at least one semester of clinical or prior experience in related field. Computer skills, Phlebotomy, EKG experience. License, Certification & Clearances Current Basic Life Support (BLS) approved curriculum of the American Heart Association (CPR and AED) program required or obtain within orientation period (five (5) days from hire). Provides the patient/family with information related to the procedure (pre-, intra-, post). Initiates the discharge process following physician approval/order for discharge. Reviews the discharge instructions with patient and family member of significant other, distributes appropriate educational materials. Discharges the patient with a responsible party or to the appropriate destination. Performs indirect patient care duties essential to maintaining the safe operation of the unit. As assigned, opens the unit each AM and locks it at the completion of the daily schedule. Checks the crash cart, AED and eye wash station daily when the unit is open and completes the appropriate checklist when applicable. Verifies the narcotic count with another RN weekly when applicable. Adheres to personal radiation protection and safety measures by wearing appropriate protective equipment and film badge when assisting with ERCP procedures. At Latrobe Hospital, prepares, assesses and discharges patients presenting to Medical SPU for blood transfusions, IV therapy and nursing service visits. Prepares patients for physician exam by assisting with undressing and positioning as needed. Administers blood transfusions per established policies. Required Qualifications Graduate of an accredited Nursing Program required. Strong leadership ability, good organizational skills, independent and critical thinking skills, sound judgment, and knowledge of legal aspects and liability of nursing practice. Strong ability to communicate complex and/or controversial topics and concepts to a wide and diverse audience. A Nursing refresher course may be required if employee hasn’t worked in an acute care setting for his / her most recent three consecutive years of professional employment. Preferred Qualifications Bachelor’s Degree in Nursing preferred. One (1) year GI Lab experience. One (1) year med/surg experience. Membership in professional specialty organization. License, Certification & Clearances Current licensure to practice as a Registered Nurse in the State of Pennsylvania required. Job Description Responsible for supervision of self-administered medication for the Assisted Living Department in compliance with Federal & State regulations and Senior Lifestyle Corporation guidelines as directed by the LPN/LVN/RN and/or the Assisted Living Director/Health and Wellness Director. Count and sign on approved form, all narcotics with approved personnel before accepting medication keys. Contact a supervisor immediately with discrepancies. Order all necessary medication and treatments from appropriate pharmacy or notify responsible party when supply is no less than 7 days. Notify supervisor when supply is less than 2 days and medicine hasn’t been received. Responsible for receiving and verifying all medicine received is accurate according to pharmacy label and MAR. Notifies pharmacy and DH&W of any discrepancies. Responsible for proper storage of all medications and ensuring all lotions, ointments and liquids stored separately from oral medications. Responsible for dating all appropriate vials or bottles at time of initial opening and following pharmacy protocol on length of time they can be used. Job Description Responsible for supervision of self-administered medication for the Assisted Living Department in compliance with Federal & State regulations and Senior Lifestyle Corporation guidelines as directed by the LPN/LVN/RN and/or the Assisted Living Director/Health and Wellness Director. Count and sign on approved form, all narcotics with approved personnel before accepting medication keys. Contact a supervisor immediately with discrepancies. Order all necessary medication and treatments from appropriate pharmacy or notify responsible party when supply is no less than 7 days. Notify supervisor when supply is less than 2 days and medicine hasn’t been received. Responsible for receiving and verifying all medicine received is accurate according to pharmacy label and MAR. Notifies pharmacy and DH&W of any discrepancies. Responsible for proper storage of all medications and ensuring all lotions, ointments and liquids stored separately from oral medications. Responsible for dating all appropriate vials or bottles at time of initial opening and following pharmacy protocol on length of time they can be used. Responsible for weekly inspections of all medications to ensure all expired medications are removed. Responsible for verifying all current MARs are compared to new MARs at change over time and notifying the supervisor of any discrepancies. Responsible for removing all discontinued medication from med cart and giving it to the supervisor. Provide personal care to residents to include, but not limited to, bathing, skin care, oral hygiene, grooming, dressing, incontinence care, and eating, etc. Provide general nursing care to include, but not limited to, vital signs, temperatures, blood pressure, and weights. Document appropriate information in the daily log as required and notify supervisor of all concerns. Answer calls and assist residents with needs as assigned. Responsible for daily cleaning of all medicine carts at end of shift and restocking as needed. Responsible for weekly deep cleaning of all med carts. Attend and participate in staff training and in-services. Must maintain accurate records to include preparation of all medical charts for new admissions and readmissions and ensures medical record is complete within forty-eight (48) hours as Policy and Procedure (P&P) and regulatory guidelines. Qualifications Level of Formal Education: A High School Diploma or General Education Degree; or one to three months related experience and/or training; or equivalent combination of education and experience Years of Experience: 1+ months of experience Type of Experience: Senior Living preferred Special Certifications: Current Medication Technician certification depending on state regulations Language Skills: Acceptable proficiency in English to perform job duties, communicate with supervisor and other employees, and respond and communicate to emergency situations. Technical Competencies: Basic computer proficiency to include Microsoft Word, Excel, and Outlook Skills and Ability: Excellent interpersonal and communications skills.
