A MSN or DNP are not required but supersede the requirement for the BSN. It is a fast-paced environment where state-of-the-art technology, nationally recognized standards of care and critical thinking skills are utilized by the all-RN staff to provide compassionate patient and family-centered care. Staff on this unit are highly proficient in pre-operative and intra-operative support as well as immediate post-operative phases of cardiovascular procedures. This includes open heart surgery, ventricular assist device implantation, and various interventional procedures including transcatheter aortic valve replacement. Acute cardiology and heart failure patients also receive care in CVICU, along with overflow medical and surgical intensive care patients. Staff expertly manage all types of invasive lines, medications, hemodynamic needs, intra-aortic balloon pumps, continuous dialysis and ultrafiltration machines, and various ventricular assist devices. Job Description Full Time Day GSH Care Management Job Overview The Care Management Coordinator is responsible for overseeing the processes and documentation that occur as evidence based practice is applied at the bedside. Directs and or completes all functions of the government and commercial utilization management process. Assesses and coordinates implementation of the discharge plan. Collaborates with the multidisciplinary team to assure effective, efficient, appropriate care in order to optimize the health status and assure a smooth transition between levels of care for patients served. Performs utilization review for patients served. Requires weekend, holiday, and off site commitment, with the exception of Hospice of Cincinnati. Job Requirements Associate's Degree or Diploma in Nursing Hires into position (internal and external) required to obtain BSN within 5 years of hire BLS/CPR (Basic Life Support for Healthcare Providers) Registered Nurse Excellent communication skills Ability to negotiate Problem solve Establish effective working relationship with members of the healthcare team and external customers Knowledge & experience in computer key boarding Professional organization membership preferred 3-4 years experience Clinical in Nursing Job Responsibilities Using current practice guidelines, assesses all patients, evaluating progress toward clinical goals and next level of care. Reassesses as clinical condition changes. Complete initial utilization review using Interqual criteria. Take follow up action to resolve the situation when criteria are not met by implementing and documenting according to the Utilization Review plan. Coordinate the development of a comprehensive plan of care through patient/family involvement in an interdisciplinary team process. Ensure the patient's health care goals are met related to evidenced based quality care. Ensure that all commercial and government payment requirements are met for continued stay reviews, and other conditions of participation. Ensure that CMS requirements are met for patient status. More about our 7S Heart & Vascular Medical Stepdown unit : The Heart Failure/7 South consists of 28 private medical stepdown beds in a family-centered environment. In addition to caring for heart failure patients, the unit specializes in the care of patients diagnosed with MI, cardiac arrhythmias, syncope and cardiomyopathy as well as the recovery of patients following ablation, pacer placement/ICD and PTA. Medical services are offered to all types of patient populations requiring cardiac monitoring, including renal, endocrine, pulmonary, oncologic, general medicine and geriatric. Other services provided within this unit include titration of multiple intravenous medications and conscious sedation for cardioversions. In addition, a select group of nurses complete special training to provide ultrafiltration for decompensated heart failure patients and left ventricular assist device (LVAD). All nurses are ACLS certified and have completed an EKG interpretation course. This is an excellent unit for nurses interested in transitioning to an ICU setting.

More about our Labor & Delivery unit: The Labor & Delivery unit is a 29-bed unit handling approximately 3100 deliveries and 2100 ambulatory patients per year. This unit delivers care to a complex range of patients varying in age from newborns to teens, young adults and adults. Within the unit, there are 12 labor and delivery beds, 4 high-risk beds, 3 post-op recovery beds, 3 surgical suites and a 4-bed triage unit. Patients are triaged, admitted, stabilized, delivered or transferred to a level III facility as appropriate.
