Business Title: Administrative Nurse Supervisor ** Amended The Bureau of Public Health Clinics (BPHC) has the mission to promote a healthy community by providing New Yorkers with the resources needed to make informed and empowered health decisions; identify and treat tuberculosis, and provide immunization and sexual health services regardless of ability to pay or immigration status . To achieve this, 8 sexual health clinics provides direct clinical services to people seeking sexual health care, and services to sex partners. The BPHC operates 8 Sexual Health Clinics throughout New York City (NYC). BPHC seeks to hire an Administrative Nurse Manager to work in one of the Sexual Health Clinics; this person will work under the supervision of the Nurse Practitioner Medical Director. DUTIES WILL INCLUDE BUT NOT BE LIMITED TO: - Obtaining medical and sexual history - Conducting physical examinations - Ordering relevant stat and microbiologic testing - Making presumptive diagnoses of STIs - Providing treatment for patients with STIs and HIV. - Providing counseling for medication abortion and dispensing abortion medications to patients in the clinic - Providing Post Exposure Prophylaxis (PEP) and Pre-Exposure Prophylaxis (PrEP) for HIV prevention - Providing all forms of contraception, including long-acting reversible contraception (LARC) - Providing vaccination for Hepatitis A and B, Meningitis, and HPV - Arranging referrals to clinics for other services as needed - Providing health education to patients - Rotating on the telemedicine service - Providing counseling and follow up for medication abortion on the sexual health Telemedicine hotline - Serving as a supervisor, including managing medication abortion and ultrasound appointments and supervision of clinicians providing counseling on medication abortion and LARC - Serving as a supervisor of clinicians providing medication abortion and LARC - Obtaining consent and dispensing medication for medication abortions in the clinics for both appointment and walk-in patients - Providing simple ultrasound evaluations for pregnancy dating and identifying IUD placements - Working closely with mental health specialists to provide comprehensive psychosocial and behavioral care - Adhering to Bureau Standards for productivity. - Performing other duties as assigned by the Assistant Commissioner and the Medical Directors. Position Summary The RN Care Manager performs the initial comprehensive assessment on admission in accordance with the Care Management Department policy, screening all patients by utilizing established tools for high risk indicators to ensure high risk patient populations receive the appropriate supportive services for discharge to prevent readmission and assess all populations for potential discharge planning needs. Provides ongoing reassessment of needs throughout the hospital course. Performs UR activity per department policy. This full-time role supports individuals with dual diagnoses of intellectual and developmental disabilities (IDD) and severe mental illness (SMI) needs by integrating skilled nursing care, crisis stabilization, transitional planning, and intensive care coordination. The RN collaborates with interdisciplinary teams to promote health, safety, dignity, and successful community integration. The RN ensures that complex medical, behavioral, and psychiatric needs are met during the transition from an inpatient setting to the community. ESSENTIAL RESPONSIBILITIES Clinical Care Screen individuals who are being considered for admission to the CRTP and the Transitional Respite Program. Active member of the "intake” team. Conduct initial and ongoing nursing assessments for individuals admitted to CRTP. Oversight of self-medication administration program, monitor therapeutic response, and document outcomes. Provide direct nursing interventions such as wound care, diabetes management, respiratory illness support, and chronic disease oversight. Monitor early signs of medical or psychiatric crisis and initiate appropriate interventions. Regularly reviews medication administration records, logs documenting vital signs, elimination, glucose readings, weight, menses, and other records as individually specified. Reports significant issues to appropriate clinicians and team members, including the need for ordering medications, renewing or updating prescription orders, and people-supported specific drug information sheets. Monitor and manage medication storage and disposal procedures in accordance with federal and state law. Develop and monitor health goals that are person specific. Review, update, and revise these documents to ensure they are reflective of the current person supported status. In compliance with OSHA mandates, monitors infection control, environmental changes, and ensures that all guidelines regarding universal precautions are appropriately followed. Order and maintain necessary first aid and medical supplies as needed. 24/7 availability via phone for consultation and basic direct care. Case Management & Transitional Support Partner with ICM teams to address both medical and behavioral health needs in service planning. Deliver health education and coaching to individuals, families, and direct support staff to promote independence and wellness. Facilitate hospital discharge and community reintegration through CTI, ensuring continuity of medical, psychiatric, and social support. Coordinate follow-up care with primary care providers, specialists, and community-based resources. Collaboration & Training Serve as the clinical liaison among providers, families, and program staff. Participate in interdisciplinary treatment planning and case conferences. Train and mentor direct support professionals (DSPs) on health-related tasks and emergency response. Maintain timely and accurate documentation in compliance with OPWDD, OMH, and DOH regulations. Qualifications: Current New York State RN license (unrestricted). Minimum 2 years of nursing experience; background in IDD, behavioral health, crisis care, or community health preferred. Familiarity with OPWDD, OMH, and DOH nursing regulations and program requirements. Strong clinical assessment, crisis intervention, and care coordination skills. Excellent communication, organizational, and documentation skills. Ability to work independently while thriving in a team-based environment. Valid driver's license preferred (travel to multiple program sites may be required). GENERAL JOB SUMMARY An exempt clinical position where the nurse practitioner (NP) is responsible for providing direct patient care. The NP participates as a leader of the skilled nursing facility (SNF) care team. Visits managed care and fee-for-service patients at skilled and long-term levels of care in designated SNFs facilities. Provides appropriate evidence-based geriatric medicine. Coordinates care with hospitalists, primary care physicians and care managers. Makes home visits as directed by the medical staff to meet patient needs and provide continuity of care. ESSENTIAL JOB FUNCTIONS Maintains privileges in multiple Nursing Homes as directed by ACA Maintains license and malpractice insurance Consults supervising attending as needed Documents patient visits electronically at least 90% of the time Participates in documentation and other quality improvement programs Available via phone weekdays 8am- 7pm and when on call. Will reviews, approves, and modifies admission orders Creates a detailed admit note for each admission within 24 hours of patient admission to SNF, including medication reconciliation Initiates/documents Advanced Directives Determines if Health Care Proxy status is correct and invoke if appropriate On weekends, takes call for admissions and see new patients within 24 hours of admission on a rotating basis with other practitioners in the program.
