Department Overview South Operating Room (Main OR) is a large 25-bed OR primarily an adult inpatient operating room with a small volume of outpatient procedures including: Cardiac/ Thoracic, General Surgery/Minimally Invasive/ Colorectal/Bariatric, Gynecology, Oral Maxillofacial, Neuro/neuro spine, Orthopedic/orthopedic spine, Otolaryngology, Plastics, Transplant, Urology, Vascular, Robotic (Urology/GYN/Oral), and Trauma. Surgical procedures on infants and children less than 18 years of age are also performed if the procedure requires specialized equipment found only at this location. The OHSU Operating Room provides around-the-clock multidisciplinary services to adult patients across their life span. Department Overview The Labor & Delivery Unit has 12 labor suites with jetted Jacuzzi tubs, a 4 bed triage area, 3 dedicated operating rooms, and a 2 bay PACU. We provide patient care for our most complex and fragile patients as well as to patients who prefer low intervention births. We have a robust midwifery practice that also promotes underwater birth. How You'll Make an Impact We are hiring Nurse Practitioners and Physician Assistants with acute care experience in ER, Urgent Care, or Internal Medicine to join our growing team serving the Portland, OR community. Our Advanced Practice Providers team with DHMTs (Medical Technicians) and are equipped with everything needed to treat common to complex injuries and illnesses through comprehensive health assessments, including diagnosis, treatment, and outcome management, for patients of all ages. Our innovative model allows us to provide definitive care in the home, including point of care laboratory studies, minor procedures, splinting, wound care, suturing, IV fluid and medication administration. We provide whole person care by facilitating timely follow up and care coordination. Our providers love working at DispatchHealth because we are a leader in delivering high-quality, in-home acute care services. In 2024, they achieved significant milestones, including completing 476,428 acute care visits, advanced care admissions, and imaging studies. Department Overview We are the Orthopedic and Spine Surgery unit as well as the Surgical Observation Unit for OHSU. Our primary patient populations include total knee and hip replacement patients, orthopedic oncology patients, spine surgery patients, and various other bone/joint patients who may be getting a revision, dealing with a fractured extremity, or a complication such as a non-union or infection. We also see a variety of observation/short stay patients who may be here for Urologic Surgery, Plastic Surgery, ENT, Emergency General Surgery, and Neurosurgery. Our unit collaborates very closely with PT/OT throughout the day to see how our patients are mobilizing and making progress. We also work together with Social Work, Case Management, Ortho and Spine Nurse Practitioners and Physicians Assistants as well as the Acute Pain Service on a regular basis to provide exceptional care for our patients. Department Overview The Care Management department provides several major services to assure safe and efficient through-put of patients during their acute hospitalization and transition to the next level of care. Our goal is to balance individual clinical and psychosocial patient/family needs with the efficient and cost-effective utilization of resources while promoting quality outcomes. RN case managers assess patients on admission for available family support, insurance resources, and potential discharge planning needs. They engage in daily multidisciplinary rounds Monday through Friday to develop and implement patient discharge plans with input from various other disciplines. The RN case manager is responsible for coordinating post-acute and discharge needs in a timely and cost-efficient manner. Using established criteria to determine appropriateness of continued hospital stay, RN case managers work with payors on obtaining needed authorizations for ongoing hospitalization and/or identified post-acute needs. The RN Case Manager collaborates with the patient and family, members of the multidisciplinary team, community resources and agencies, and payors to ensure the patient’s progress and level of care is appropriately determined. Intensity of service is dependent on individual patient needs as assessed by the case manager. RN case managers practice robust utilization and resource management. RN case managers also frequently lead multidisciplinary and family care conferences involving complex care situations. They contribute to meeting OHSU’s strategic plan of safe length of stay reduction and reduction in readmission rates. Case Management is represented on every adult and pediatric unit and in the Emergency department. Each Case Manager has an office with a workstation near the unit they are assigned to. Communication factors: Case Managers are available 7 days a week between the hours of 0700-2300. They are available by pager, telephone, e-mail, and approved messaging applications. Admit/ED Nurse Case Manager: In addition to the above basic CM functions, also provides the following – Assuring accurate patient status and admission order for pts entering via ED, and OR, 7 days per week Reviews ED Observation unit patients for meeting observation criteria and completes UR using established criteria. Round with NP in ED OBs by 10am. Effectively communicates reviews with Patient Placement Office. Follows up in collaboration with unit CM on admission status and completeness of Utilization review on admission. Performs discharge planning for patients in the Emergency department Assesses patients potential to DC from ED with community care coordination (PCP location, follow up appointments, medication assistance, SNF, long term care, DME, Memory or Foster care) Back up for weekend/EVE Case Managers. Follows current data tracking procedures Reviews transfer center patients and advise physicians on acceptance of potential transfers. Preforms Utilization Review for each patient admitted without a review during weekend hours using established medical necessity guidelines. Reviews order/classification discrepancies and take actions to resolve the discrepancy. Discusses cases with providers and Case Managers as needed, including attending physicians and escalation to the Care Management Physician Advisor when indicated. Assesses for and tracks potentially avoidable hospital days. Educates providers regarding documentation requirements that support medical necessity determinations. Delivers Condition Code 44 notices, Observation notices, and Medicare Important Messages in the absence of sufficient clerical support. Documents according to departmental policy. Leads the effort to assure compliance with CMS and other insurance regulations related to Utilization Review. Evening Shift RN Case Manager Provide nurse case management support to patients, families, and staff in both the inpatient and emergency department settings for the evening shift 7 days per week. Receives referrals and hand offs from day shift case managers. Complete hand off of information to day shift case managers. Trouble shoot and resolve any discharge delays or barriers carrying over into evening shift. Other functions as assigned. RN Case Managers are responsible to be aware of and comply with regulatory requirements of DNV, Medicaid, Medicare and Oregon Nurse Practice Act. Schedule/Shift : FT, Monday-Friday, Day Shift Territory/Location: Portland \n Responsibilities: Provide skilled nursing care and support to hospice patients in accordance with state guidelines and hospice policies. Perform comprehensive patient assessments, develop individualized plans of care, and revise care plans as needed. Manage an assigned caseload of patients, coordinating care across disciplines to ensure quality and continuity. Complete admissions by performing initial assessments, verifying hospice eligibility, educating patients/families, and initiating the plan of care. Administer medications, treatments, and procedures as prescribed. Educate patients and families on symptom management, disease progression, and end-of-life care. Serve as the primary point of contact for patients, families, and healthcare providers regarding care coordination and clinical updates. Maintain accurate, timely clinical documentation in compliance with regulatory and organizational standards. Participate in interdisciplinary team meetings to ensure collaborative care planning. Support patients and families emotionally, advocating for patient comfort, dignity, and informed decision-making. Qualifications: Graduate of an accredited School of Nursing; current RN licensure in the state. Current CPR certification. Minimum one (1) year recent nursing experience; hospice or palliative care experience preferred. Experience with case management and/or hospice admissions strongly preferred. Strong clinical, assessment, organizational, and communication skills. Ability to work independently and collaboratively in home and facility settings. Capable of meeting the physical demands of patient care (e.g., lifting, positioning, standing, walking). Job Summary: The Home Health Access RN provides quality, cost effective clinical coordination and transitions to the home from the acute care, emergency care settings, and clinic or primary care. The Home Health Access RN manages patients with routine and complex discharge planning needs through collaboration with inpatient and/or SNF care coordination. The Home Health Access RN also serve as expert resource consultants and educators for physicians and other health care team members in transitions of care to the Home Health Program, coordination of internal and community resources, and support the evaluation and improvement of systems of care to support the optimal utilization of home health resources, while maintaining quality of patient care. Essential Responsibilities: Quality and Service: Providing an exceptional patient care experience, consults with patients, families, physicians and hospital staff to gather appropriate clinical and demographic data to ensure a smooth transition from hospital to home health care. Provides an exceptional patient care experience that includes appropriate patient care planning according to the needs of the patient and family/caregiver members. Collaborates with insurance companies, other managed care entities, DME providers and others to provide assistance to field staff in the Case Management process to provide an exceptional patient care experience. Promotes and maintains continuity of care for patients being discharged from the hospital, clinic or SNF to home care services. Communication and coordination: Triages incoming referrals based on the patient needs. Determines initial level of care and services ordered and assesses the need for additional services for patients referred to Home Health. Provides oversight and supervision for LPN and Referral Coordinators to support timely admission to the home health program. Utilize case/family conferences and consults to develop these care plans as needed. Implementing care plans by ordering, brokering, and advocating for the patient and family, while educating the patient, family, and health care team about options and alternatives. Notifies CCS Managers of any discharges of patients with complicated social and medical issues that delay initiation of home care. Education, documentation and program development functions: Educates patients, their families and the facility staff about the services and products provided by Kaiser Continuing Care Services. Make recommendations to modify the plan of care to expedite a safe transition of care, reduce risks, and enhance patient outcomes. Accurate documentation of all patient/caregiver contact and communication with clinic staff, PCPs, hospital care coordination in both the agency record and/or in Healthconnect to assure CCS staff are informed of all interventions undertaken in patients behalf. Ensures systematic and on-going contact with hospital staff and other Kaiser Permanente Departments (e.g. GLTC, DME, Hospital Care Coordinators, Clinic Social Workers, Clinic case Managers, Pharmacy, Home Infusion Program, Membership Services, etc) to share information and ensure safe transitions of care. Participates in quality and utilization management activities (e.g. Chart audits). Participates in continuing education to incorporate and maintain up to date knowledge and best practices in customer service and exceptional clinical care for the home setting. Completes Medication Reconciliation for therapy only cases per regulations and standards. Assists RN team with optimizing the patients home care plan by providing: Resource, support, and advice for home care staff Physician communication and support. Supply and DME ordering as needed. Assistance with paneling patients with physicians. Regulatory: Ensure regulatory and compliance standards are met for admission to home care programs. Understands Medicare guidelines and is responsible for adherence to all state and federal regulations and JCAHO standards. Productivity: Supports productivity for field staff and participates in productivity work within the CCS department. Covers patient advice calls as needed. Participate in program call duty as needed. Conducts patient home visits as required to assist the program in meeting patient needs. Perform other duties as requested.
