JOB DESCRIPTION SUMMARY The hospice aide works under the supervision of a registered nurse and performs various tasks as assigned in accordance with the patient’s plan of care to meet the patient's personal needs and to promote comfort. The hospice aide is responsible for reporting and documenting observations and care performed. The hospice aide will be assigned in a manner that promotes quality, continuity and safety of a patient's care. ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES Responsibilities of the hospice aide include, but are not limited to, the following: 1. Performing personal care needs after given a patient's assignment and attending to his/her requests promptly. If unable to perform a certain task, report to the Case Manager immediately. 2. JOB: LPN Advanced Center for Nursing and Rehab is actively hiring Licensed Practical Nurse (LPN), for the night shift, 11pm-7am, part-time for our Skilled Nursing Facility in the New Haven area of CT. DUTIES: Order prescribed medications, supplies, and equipment as needed Record health details of residents; including vitals & temperature Carry out direct care to residents based on the care plan in accordance with the residents needs, nursing procedures, facility policy, and nursing standards of practice Administering medications and treatments to residents as needed Supervise Certified Nursing Assistants Perform assigned duties including wound care, treatments, and procedures Accurately document observations, interventions, and evaluations pertaining to resident Maintain confidentiality and rights of residents, family members, employees, and facility information in compliance with HIPAA policies REQUIREMENTS: Valid CT State LPN license Excellent leadership and communication skills Long term care experience preferred BENEFITS: Competitive pay rate and benefits $750 sign on bonus after 90 days for full-time $375 sign on bonus after 90 days for part-time Weekly pay Direct Deposit Great working environment ABOUT US: Advanced Center for Nursing and Rehab is a 226-bed skilled nursing and rehabilitation facility situated in New Haven, Connecticut.
