We're seeking experienced nurses with a passion for community health education and engagement. You'll utilize community resources to tailor services that meet the unique needs of individuals and families within their homes and communities. Assessment and Care Planning: Conduct initial and ongoing assessments of patients' health status. Develop and implement individualized care plans based on these assessments to meet the patient's needs. Medication Administration: Administer medications and treatments as prescribed by the physician. Monitor and assess the patient for any adverse reactions or side effects. Wound Care: Provide wound care management, including dressing changes, infection control, and monitoring healing progress. Patient Education: Educate patients and their families about their medical conditions, medications, and treatment plans. Teach them techniques for self-care and management of their health at home. Monitoring and Evaluation: Monitor patients' progress and condition regularly. Evaluate the effectiveness of the care plan and make necessary adjustments based on patient outcomes. Knowledge of nursing and medical practices, procedures, regulations and guidelines pertaining to home care, including OASIS. Complies with accepted professional standards and practice.
