POSITION DESCRIPTION TITLE: LPN (OSHA Classification 1) FLSA: Non-exempt POSITION SUMMARY: Responsible for the overall operation and optimal quality of care for the residents on the assigned unit. Follows all health, sanitary and infection control policies and maintains established standards of practice set forth by the facility's administration and Nursing policies and procedures. Ability to perform physically demanding worked involving frequent bending, stooping, turning, stretching, and reaching above the shoulders are involved. ⦁ Must be able to see and hear or use prosthetics/equipment that will enable these senses to function adequately to assure that the requirements of this position can be fully met. ⦁ Must be able to lift a minimum of 50 lbs. Communicates essential information to appropriate parties. ⦁ Makes rounds on assigned residents at least once every two hours. ⦁ Collaborates with Care Plan Coordinator to develop and maintain nursing care plans and update these plans on a regular basis and as the resident's condition changes. ⦁ Attends and participates in interdisciplinary resident care conferences as requested. ⦁ Assists the attending physician with rounds and special procedures as needed. ⦁ Participates in the yearly written evaluations of nursing assistants and documents accurately their strengths, weaknesses and the steps that need to be taken to improve their performance. ⦁ Assists Staff Development with orientation and on-the-job training for new nursing personnel as needed. ⦁ Collaborates with nursing supervisor in documenting disciplinary concerns for nursing assistants on designated shifts, including reprimands, oral and written warnings, and corrective action recommendations. ⦁ Notifies nursing supervisor, resident’s physician, and family of a change in the resident’s condition or an accident/incident involving the resident. ⦁ Assumes any duties that are assigned by the RN Supervisor/ADNS to provide optimal quality resident care. ⦁ Performs other related duties and attends meetings as may be deemed necessary by Supervisor / Administrator and or Director.
