Monitor participants health status such as vital signs, weights, skin condition, appetite and changes in functional ability and report pertinent information to clinical supervisor in a timely manner. Collaborate with multidisciplinary teams in care planning for participants. Monitor appetite and look for changes in functional ability and skin condition. Monitor/assist participants in a courteous manner with meals and helps with feeding, as necessary. Report any variance, accident, or unusual occurrence to the appropriate personnel. Assist with loading and unloading of participants from vehicles. Assist the transportation department with participants’ take-home supplies. Serve as a bus attendant on company vehicles while InnovAge drivers transport participants to/from center or medical appointments supporting participants while in transit and accompanying participants in and out of facilities.
