Hospice RN Inpatient Admission Coordinator Affinity Care of Ohio Position Summary The Hospice RN Inpatient Admission Coordinator serves as the primary connection between Affinity Care of Ohio, referral sources, patients, and families. This role is responsible for building strong community relationships, identifying hospice-appropriate patients, coordinating admissions, and ensuring a compassionate, seamless transition into hospice services. The ideal candidate is a skilled communicator with a deep understanding of hospice philosophy and a commitment to patient-centered care. Key Responsibilities Referral & Relationship Management Build and maintain strong relationships with hospitals, skilled nursing facilities, physicians, social workers, and other referral partners. Educate healthcare professionals about hospice philosophy, services, eligibility criteria, and Affinity Care of Ohio’s programs. Respond promptly to new referrals and inquiries, ensuring timely follow-up and exceptional customer service.
