SUMMARY: Under the supervision of a licensed Physician, performs a variety of duties to encompass, but not limited to the care and management of orthopedic patients. Works in a collaborative team environment that supports the academic mission to train orthopedic residents. Provides inpatient and emergency department consults, pre and post-operative management of patients, assistance with procedures, wound care, and other patient care duties. Obtains and records detailed medical histories. Performs and records complete and detailed physical examinations. Records all pertinent patient information in the progress notes within the medical records. Assists in the preparation of discharge summaries. Initiates life saving measures in emergency situations until aid arrives. Initiates patient orders for admission as directed by the physician and in accordance with the hospital procedure. Assesses patient status, participates in the development of patient care management plans through communication and collaboration with the Registered Nurses, attending physicians, residents, and other members of the interdisciplinary team. Evaluates and cares for patients who present in the Emergency Room with orthopedic problems. Provides wound care, including cleaning and debridement of wounds, as well as dressing changes. An extremely high level of analytical ability in order to apply post-surgical training to the treatment of patients. Good interpersonal skills in order to deal effectively with patients and nursing personnel. EXPERIENCE: Experience in General Orthopedics, Surgery and Emergency Room procedures preferred but not required. Job Summary The Providence Center ACT Engagement and Diversion Team is looking for a motivated, team-oriented Registered Nurse, to help assess and facilitate the tasks and duties listed below for adults with co-occurring conditions, medical complexities, and who are also dealing with unstable living arrangements. Experience with the chronically homeless and addictions populations a plus, and within our new "CCBHC" (Certified Community Behavioral Health Clinic") capacity. Duties & Responsibilities Triage and assess assigned consumers on the team. Assess and evaluate client health care data collected through interviewing, observation, record review and team input. Develop an individualized consumer plan of care. Perform integrated care in accordance with appropriate medical protocol. Identify expected outcomes of care, individualized to the client, with the ultimate goal of improving client’s overall health. Implement the consumer plan of care. Evaluate the consumer's response to the plan of care and update the plan based on consumer's response. Provide support to consumers regarding daily living issues, conducting community and home visits as necessary. Instruct consumers and families concerning the disease process, plan of treatment and rehabilitation course, using available teaching materials when appropriate. Educate consumers on the various medications on the expected effects and side effects. Question any aspect of the consumer's treatment regimen which endangers the level of wellness. Administer medications as per prescriber orders (Injections and PO). Implement appropriately prescribed medical regimen while following the 6 rights of medication administration. Monitor lab results and complete related documentation Ensure that externally prescribed medications (from outside providers i.e. PCP) are accurately documented in EMR. Liaise with treatment team and pharmacy regarding medication changes to ensure clients receive accurate and most current prescribed medications. Coordinate activities regarding consumer hospitalization. Follow up with clients post-hospital discharge to provide support and education regarding recommendations resulting from inpatient stay. Complete medication reconciliations, review hospital discharge paperwork, and communicate any changes or significant findings to treatment team, including MD. Collaborate with the staff members in providing care to consumers. Perform recordkeeping in accordance with Agency requirements. Complete initial and annual assessment forms. Maintain cooperative relationships with consumers, staff and the public. Communicate with various local agencies, medical facilities, and other departments within TPC, including pharmacy. Attend trainings, case presentations and conferences as needed. Perform duties in accordance with departmental goals, objectives and standards. Serve on appropriate committees. Functions as intake liaison to other VNA departments and referral sources Accepts referrals directly from hospitals and community referral sources. Accepts all referral-related patient medical and insurance information. Registers patients and monitors referral source activity. Oversees insurance verification and prior initial authorization. Serves as clinical resource for clerical intake staff. Acts as a gatekeeper by screening patients for eligibility risk management and safety at home. Enters original orders. Performs all other related duties as assigned. Demonstrates specialized level of knowledge and thorough understanding of the practices/procedures related to home health care equipment/supplies home care nursing practices medical insurance requirements and reimbursement practices.
