Must reside in Mountain or Pacific time zone and hold multistate nursing license in state of residence Position Overview We are seeking a highly motivated and experienced Registered Nurse (RN) with a strong background in chronic condition education and telephonic 1:1 member coaching. This is a remote, full-time position (40 hours/week) providing health coaching, assessment, and care support to members with chronic health needs. The RN Health Coach Consultant plays a key role in helping members understand their conditions, adopt healthier behaviors, and achieve optimal health outcomes through a collaborative approach. Applicants must be able to work independently in a remote environment while providing exceptional member support, education, and clinical insight. Work Schedule • Monday–Friday: Standard business hours supporting PST or MT time zone (8:00–4:30 PM or 8:30–5:00 PM), including two evening shifts per week (11:30 AM–8:00 PM PST or MT) • Occasional Saturday support may be required based on business needs • One late-Friday rotation every 4–6 weeks • Remote Work Primary Responsibilities Member Assessment & Engagement: • Conduct clinical assessments and review health data to evaluate member needs and program eligibility • Assess member readiness, health literacy, and technology capability • Engage members in chronic condition support and personalized coaching Health Coaching & Lifestyle Modification: • Provide individualized telephonic health coaching using motivational interviewing • Support sustainable behavior change and promote positive health outcomes • Empower members to make informed healthcare decisions Clinical Quality & Care Coordination: • Apply clinical guidelines, policy criteria, and regulatory standards • Consult with supervisors and Medical Directors to address barriers • Identify and escalate quality-of-care concerns • Provide accurate health information to support understanding Monitoring, Evaluation & Documentation: • Develop and maintain individualized care plans • Document all interactions per regulatory and accreditation requirements • Monitor progress and adjust plans to meet member needs Required Qualifications: • Registered Nurse must hold an unrestricted nursing license and reside in a compact state with multi-state/compact privileges and have the ability to be licensed in all non-compact states and must reside in the Mountain or Pacific Time Zone. • 3+ years of acute adult Medical-Surgical RN experience • High-speed, hard-wired internet connection • Proficiency with Microsoft Office (Word, Outlook, Excel, Teams, PowerPoint), multi- system navigation and keyboarding • EMR documentation experience • Excellent communication skills • Ability to work independently and collaboratively Preferred Qualifications: • Telephonic chronic condition coaching experience • Managed care experience • EPIC EMR familiarity Education • Associate Degree in Nursing (ADN) required • Bachelor of Science in Nursing (BSN) preferred Anticipated Weekly Hours 40 Time Type Full time Pay Range The typical pay range for this role is: $60,522.00 - $142,576.00 This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors. Job Description Full-time (40 hours/week) · Fully Remote Position Schedule options: Standard schedules available; eligibility for 4x10 schedule after successful completion of first 90 days (post onboarding) and performance expectations. About the Role As a Devoted Preventative Primary Care Provider, you’ll be part of a mission-driven, interdisciplinary clinical team focused on improving the health, dignity, and quality of life of our members. You’ll meet patients in the first step of their primary care journey with Devoted Medical. You’ll conduct 1 hour comprehensive clinical assessments, support chronic condition management, and emphasize preventative and proactive care. You’ll collaborate closely with primary care providers, specialists, pharmacists, nurses, care coordinators, caregivers, and families to deliver coordinated, member-centered care. This role sits at the intersection of preventative care, primary care and complex care management — it is not a traditional primary care panel role, and it’s not a one-time Annual Wellness Visit role. You’ll develop a relationship with your members while working in a supportive, tech-enabled environment designed to optimize clinical time and connection. What You’ll Do Conduct primarily telehealth video visits, with limited and occasional in-home visits to members in your local area. Perform comprehensive assessments and provide evidence-based care focused on prevention, chronic condition support, medication optimization, and health maintenance. Identify and address care gaps, coordinate with external and internal care team members, and contribute to individualized, whole-person care plans. Utilize integrated technology and AI-enabled workflows to reduce administrative burden and maximize meaningful member interaction time. Communicate with empathy and clarity, fostering trust and confidence with members and caregivers. Required Skills & Experience Active Nurse Practitioner (APRN/NP) or Physician Assistant (PA) license. 3+ years of outpatient clinical practice, ideally in primary care, family medicine, internal medicine, or geriatrics. Active and clear NP/PA license in at least one of the following states: AL, AZ, AR, CO, FL, GA, HI, IL, IN, KY, MS, MO, NC, OH, PA, SC, TN, TX (must be willing to obtain additional licenses within the first 90 days — support and reimbursement provided) Active BLS certification at time of hire. Comfort with delivering care via video telehealth and using electronic documentation systems. Preferred Experience Familiarity with managed care models, including STARS/HEDIS and identification of care gaps. Experience performing comprehensive or preventive care visits with Medicare populations. Experience supporting older adults and/or individuals with complex medical or social needs.
