Summary : Job Profile Summary The RN Clinical Nurse provides patient-centered, specialized, evidence-based nursing care across the continuum through an interprofessional approach to treatment, research, education, and advocacy. Through collaborative practice with members of the care team, is responsible for patient outcomes that meet the high quality of care provided by ThedaCare. Job Description: SCHEDULE: (1) 12-hour shift every 2 weeks required Weekend and Holiday requirements discussed in the interview Orientation will have to be either part-time or full-time status UNIT DETAILS (Behavioral Health- Neenah Hospital): This 19 bed unit serves a great purpose in the community treating various health needs. Our compassionate team is comprised of Psychiatric hospitalists, RN's, CNA's, Occupational Therapists, mental health therapists, utilization specialists, and administrative professionals. This environment allows for collaboration and learning on a daily basis. Our team is dedicated to the highest quality of care and fosters great learning opportunities. KEY ACCOUNTABILITIES: Utilizes the nursing process, evidence-based practice, and specific competencies to assess the physical condition and nursing needs of patients, and develops a plan of care in a collaborative practice with the patient and interprofessional team. Plans for the care needs of the patient in collaboration with the interprofessional team to provide the highest quality of care and clinical outcomes. Demonstrates clinical expertise in the provision of care in the clinical specialty assigned, and performs all functions of the professional clinical nurse (RN), which are age appropriate, developmentally sensitive, and culturally specific. Identifies ways to improve the patient’s experience of care, streamline care processes, and lower costs while promoting quality to improve patient, family, and team member satisfaction. Contributes to a professional environment that encourages mentoring, engagement, and development to retain expert clinicians. Provides consultation and maintains positive relationships with physicians and other interprofessional team members, collaborating to problem solve and improve patient care. Demonstrates, anticipates, and proactively manages risk to prevent crises. 8. Performs skillfully in life threatening emergencies, matching demands and resources during crises situations. QUALIFICATIONS: Current Wisconsin RN Licensure American Heart Association Healthcare Provider Basic Life Support (BLS) BSN Completion: Upon hire, any non-BSN nurse with less than one (1) year of licensed Registered Nurse experience will be required to obtain a Bachelor of Science in Nursing (BSN) within five (5) years of hire. This education requirement is grandfathered for employees hired prior to February 1, 2026, who may continue to meet the requirement with an Associate Degree in Nursing (ADN). PHYSICAL DEMANDS: Ability to move freely (standing, stooping, walking, bending, pushing, and pulling) and lift up to a maximum of Fifty (50) pounds without assistance Job classification is exposed to blood borne pathogens (blood or bodily fluids) while performing job duties Manual dexterity and hand-eye coordination to perform patient care procedures WORK ENVIRONMENT: Frequent exposure to sharp objects and instruments Occasional exposure to moving mechanical parts, fumes or airborne particles, toxic or caustic chemicals, and risk of electrical shock Occasional high noise level in work environment Standing and/or walking for extended periods of time Transporting, transferring, positioning patients and/or equipment from one location to another; little likelihood for injury if proper body mechanics and procedures are followed Possible exposure to communicable diseases, hazardous materials, and pharmacological agents Occasional contact with aggressive and or combative patients. Job Description Summary Hospital Sisters Health System (HSHS) is seeking a Care Manager-RN to join our team. In collaboration with the patient/family, social workers, physicians, interdisciplinary team, and payer representative, the Care Manager-RN ensures patient progression through the continuum of care in an efficient and cost effective manner. Identifies, initiates and manages optimal care coordination and discharge planning to develop patient flow/throughput to enhance continuity of care, smooth and safe transitions, patient satisfaction, patient safety and length of stay management. Provides support and expertise through comprehensive assessment, planning, implementation and overall evaluation of individual patient needs. Monitors the provision of cost effective care to ensure care is given in the most clinically appropriate setting. Facilitates compliance with regulatory requirements and organizational performance standards, including demonstration of core competencies. Wisconsin: RNs may practice with a valid multi-state RN license as outlined in the Nurse Licensure Compact rules. Case Management certification (ACM, CCM, RN-BC) is preferred. Basic Life Support Certification (BLS) is required within 60 days of hire.
