ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES Patient Care 1. Completes an initial, comprehensive and ongoing comprehensive assessment of patient and family to determine hospice needs. Position: Regional Referral Center RN Department: Regional Referral - Logistics Facility: 1611 S Main St, Dayton, OH Shift: Full-time Nights 7:00pm-7:30am/ 72 Hours Per Pay This position consists of the responsibilities of the System Access Center Coordinator and the System Patient Logistics Nurse Coordinator. The System Access Center’s RN will be crossed trained to oversee and support all roles within the RRC Access Call Center. The Nurse’s primary focus will be on the clinical assessment and the identifying the needs of the complex transfers. The RN will provide leadership and guidance to the Access Center coordinators. The Clinical nurse can share her critical expertise and provide direction for the Access coordinators. This position includes broad customer relations skills and will serve as the initial point of contact for internal and external customers for the Regional Referral Center. The RN will communicate and can dialogue with the referring and accepting physicians in order to comprehend and plan for the needs of this complex transfer. He/She has the clinical knowledge and skill to suggest and drive the admission process. The System Access Center RN will provide guidance, leadership, and clinical expertise for the System Access Center Coordinator (ASCC). The ASCC has the responsibility for coordinating all calls for Premier Health including all paging requests, consults, direct admits, and transfers from internal and external customers. The Access Coordinator will enter the on-call schedule for the various providers into the approved schedule data base as well as maintain it for any incremental changes. The Clinical Nurse will manage and guide the Access Center Coordinators. The Clinical Nurse also has the responsibility to oversee the process for all “alerts” across the system. These include: Stroke, AMI, hip fracture and sepsis alerts. This requires expertise in multiple applications to ensure proper notification to all caregivers, including physicians, nurses, medical imaging, etc. to ensure quick response to critical patients. The knowledge and critical thinking skills processed by the Clinical RN will ensure professional oversite and coordination of the management of the “alerts” across the Premier health System. The System Patient Logistics Nurse Coordinator has the responsibility of coordinating the movement of patients throughout the entire Premier Health System and outside transfers into Premier Health. The nurse will utilize her/ or his clinical expertise and critical thinking skills to ensure a seamless transfer process for both Internal and External customers. The nurse will gather medical information pertinent to the level of care which will allow her to guide the referring and accepting physician to make the best choice for appropriate level of care for the patient. The nurses’ understanding of the ‘comprehensive plan of care’ will ensure that the patient is placed on the level of care necessary to ensure delivery of quality safe care and provide a positive patient experience. The Patient Logistics nurse will be cross trained in Central Logic application to facilitate rapid bed placement, monitor bed placement by utilizing the EPIC bed boards for the system, work with physicians, nursing leadership, and EVS to expedite discharge of patients. These functions are designed to decrease length of stay, and decrease patients leaving without being seen, ultimately increasing patient satisfaction/experience. The nurse will work proactively with the referring facilities to drive emergent and non-emergent referrals to all Premier Health Facilities. The nurse will continually evaluate capacity needs and will work collaboratively with the Regional Access Center and Regional Transportation to ensure a seamless transfer process.
