Position Overview: The Case Manager focuses on clinical quality and resource management resulting in defined clinical satisfaction and financial outcomes. They coordinate patient care to a population of different ages within the Singing River Health System. Expectation is for all performed duties to be in accordance with Singing River Health System procedures and policies, accreditation organization, and governing guidance and publications for health care employees. Case Manager must be able to work effectively under pressure, multitask and be self-directed. Case Managers must have strong written and oral communication skills, analytical and critical thinking skills, conflict management and team-building skills, and strong organizational skills. The Case Manager must have the ability to perform numerous tasks simultaneously with minimal errors. Case Managers will participate in the daily discharge planning meetings: addressing barriers that may inhibit discharge; targeting successful transition from one level of care to the next. Case Managers will work to create a seamless line of communication between the inpatient care management team and outpatient case management team.
