Description Status: 40hr Nurse Navigator The Nurse Navigator serves as a dedicated point of contact for referring medical providers, patients, and caregivers, offering guidance and resources to help navigate clinical and supportive care services across the health care continuum. This role may include coordinating patient appointments and providing education to patients undergoing cancer treatment. Essential Functions and Responsibilities: Assist patients in understanding their diagnosis, treatment options and available resources. Maintain a working knowledge of cancer disease processes, treatments, side effects and their management, prognoses, and complications. Take responsibility for assuring that safe patient care is maintained including following established protocols and use of tools provided by the organization to ensure patient safety. Serve as an essential link between patients and all other care providers. Facilitate appointments for consults and support services within established service standards (labs, diagnostic testing, referrals, etc.). Support multidisciplinary involvement in the plan of care (Oncology Rehab, Dietitian, specialty medical providers and primary care providers). Educate patients and their family, providing educational tools (JCCC education binder). Maintain the JCCC education binder materials. Follow cancer patients through the care continuum, including detection, diagnosis, treatment and survivorship. Create treatment plans for each patient receiving treatment, providing a copy to the patient and their primary care provider. Create treatment summary post patient treatment, providing to patient and their primary care provider. Collect data, track outcomes and support the navigation program progress. Document patient interactions in a complete and useful manner. Coordinate outreach efforts to establish and maintain positive working relationships with key customers, including medical providers, office staff, diagnostic staff, nursing, social services, community resource agencies, the patient and the caregivers Performs other duties as assigned. Complies with local, state, and federal laws and regulations. Requirements License Requirements: Maintains current Registered Nurse licensure for the State of Maine. Educational, Skills, Experience Requirements: Maintains current BLS certification. Maintains or obtains certification in navigation, either generally or in an oncology specialty.
