Surgery Scheduler: · Familiar with ICD-9/10 coding and CPT coding. · Coordinates physician surgery schedules between OR and office. · Proficient in computer and internet skills to access payers websites, perform surgery scheduling on multiple systems, verify patient eligibility for benefits, and to obtain surgery pre-certifications. · Provides appropriate pre- and post-surgery information, organization of surgery scheduling management. Referral Liaison · Meets with patients both in person and by phone and is the point person in their coordination of care. · Works closely with insurance companies and makes referrals according to their guidelines and policies. · Coordinates referrals with other provider’s offices in a timely manner. · Implements and follows a tracking system for all referrals. · Processes requests for physical forms/immunization forms for parents, daycares, and schools as time allows. Essential Job Duties • Completes comprehensive assessments of members per regulated timelines and determines who may qualify for care management based on clinical judgment, changes in member health or psychosocial wellness and triggers identified in assessments. • Develops and implements care coordination plan in collaboration with member, caregiver, physician and/or other appropriate health care professionals and member support network to address member needs and goals. • Conducts telephonic, face-to-face or home visits as required. • Performs ongoing monitoring of care plan to evaluate effectiveness, document interventions and goal achievement, and suggest changes accordingly. • Maintains ongoing member caseload for regular outreach and management. • Promotes integration of services for members including behavioral health, long-term services and supports (LTSS), and home and community resources to enhance continuity of care. • Facilitates interdisciplinary care team (ICT) meetings and informal ICT collaboration. • Uses motivational interviewing and Molina clinical guideposts to educate, support and motivate change during member contacts. • Assesses for barriers to care, provides care coordination and assistance to member to address concerns. • May provide consultation, resources and recommendations to peers as needed. • Care manager RNs may be assigned complex member cases and medication regimens. • Care manager RNs may conduct medication reconciliation as needed. • 25-40% estimated local travel may be required (based upon state/contractual requirements). Required Qualifications • At least 2 years experience in health care, preferably in care management, or experience in a medical and/or behavioral health setting, or equivalent combination of relevant education and experience. • Registered Nurse (RN). License must be active and unrestricted in state of practice. • Valid and unrestricted driver's license, reliable transportation, and adequate auto insurance for job related travel requirements, unless otherwise required by law. • Understanding of the electronic medical record (EMR) and Health Insurance Portability and Accountability Act (HIPAA). • Demonstrated knowledge of community resources. • Ability to operate proactively and demonstrate detail-oriented work. • Ability to work within a variety of settings and adjust style as needed - working with diverse populations, various personalities and personal situations. • Ability to work independently, with minimal supervision and self-motivation. • Responsiveness in all forms of communication, and ability to remain calm in high-pressure situations. • Ability to develop and maintain professional relationships. • Excellent time-management and prioritization skills, and ability to focus on multiple projects simultaneously and adapt to change. • Excellent problem-solving, and critical-thinking skills. • Strong verbal and written communication skills. • Microsoft Office suite/applicable software program proficiency, and ability to navigate online portals and databases. PURPOSE AND SCOPE: The professional registered nurse Outpatient RN CAP 1 is an entry level designation into the Clinical Advancement Program (CAP). This position is accountable and responsible for the provision and coordination of clinically competent care including assessment, planning, intervention and evaluation for an assigned group of patients. Assists and supports quality management systems and performance improvement initiatives . Works independently, organizing work to meet established deadlines and records. Maintains supply inventories. Trains new employees and acts as preceptor for laboratory students. Operates laboratory information systems, personal computers, and instrument-related computer software. Provides exceptional customer service to internal and external customers. Schedule: Friday-Sunday 0930-2200, position must also occasionally complete phlebotomy General Requirements: Bachelor's degree in Medical Technology or in Chemistry, Biology or related field. One-year clinical internship in a National Accrediting Agency for Clinical Laboratory Science (NAACLS) accredited clinical laboratory science program or approved alternate career path that meets certification eligibility requirements. Certification in one of the following: Medical Laboratory Scientist (MLS) or Medical Technologist (MT) by the American Society of Clinical Pathologists (ASCP). Medical Technologist (MT) by the American Medical Technologists (AMT). Medical Technologist (MT) by the American Association of Bioanalysts (AAB). A person may be hired into this position if he/she has completed the educational requirements and is eligible for the certification exam. The exam must be successfully completed within six (6) months of hire. Proof of completion of Mandatory Reporter abuse training for child and dependent adult abuse within three (3) months of hire.
