Job Description: Summary: Under the guidance of the Director of Nursing the incumbent of this position will oversee the nursing staff to provide the professional nursing care for the residents of the long-term care facility in accordance with the guidelines set down by the company and the government entities who oversee regulation of long-term care facilities. Essential Duties and Responsibilities: 1. Coordinates and monitors the daily activities of the nursing services department staff 2. Maintains consistent application of EverTrue policies; Participates in coaching/counseling of nursing service personnel as necessary 3. Participates in the development, maintenance and updating of written policy and procedures that govern the day-to-day functions of the Nursing Services Department 4. Performs administrative duties, including, but not limited to workforce performance evaluations, determining work assignments and scheduling, medical record documentation, completion of RAI/MDS, and other written and oral reports as required 5. Participates in direct resident care when necessary 6. Participates in Committee meetings as required 7. Functions in the capacity of the Director of Nursing, Charge Nurse, and /or MDS Coordinator in his/her absence and as assigned 8. Assists with in-service training of the nursing services, department personnel 9. Participates in survey (inspections) made by authorized agencies 10. Maintains a safe work environment; Practices good infection control procedures; Must demonstrate, upon request, the fundamentals of disaster preparedness as outlined in the Disaster Preparedness Plan 11. Maintains the confidentiality of all pertinent resident and employee information to assure their privacy is protected 12. Conducts rounds on neighborhood(s)/unit(s) to check on residents and staff to assure appropriate and safe care delivery; Identifies issues or concerns and intervenes when necessary 13. Upon resident admission, ensures coordination and timely completion of the admission process 14. Reports and supports investigative protocols as it relates to staff or resident incidents and/or accidents in accordance with state and federal regulations 15. Complies with applicable federal, state and local laws, regulations and standards Qualifications, Knowledge, Skills & Abilities: To perform this position successfully, an individual must be able to perform each essential duty satisfactorily. Graduate of an accredited/approved school of nursing Two+ years of related supervisory experience in a geriatric environment Must be CPR certified Must hold a current state applicable RN nursing license; BSN preferred Demonstrates the initiative and the ability to work independently and prioritize tasks Must be able to follow direction written or oral Must have patience, tact, flexibility, a cheerful disposition, enthusiasm as well as the willingness to handle difficult situations Must have legible handwriting, good telephone and filing skills Physical Requirements and Working Conditions: The physical activities and demands described here are representative of those an employee encounters while performing the essential functions of this job. Are you living your life's work? LOCATION: Elizabethtown, PA - Norlanco Urgent Care HOURS: Full Time (36 hours per week). Three 12-hour shifts per week. Every third weekend and rotating holidays. Position Summary: To provide nursing services to patients at a provider’s office. Patient care includes care to patients in following age group: pediatric infant to 12 years, adolescent 13 to 17 years, adult 18 to 64 years and geriatric over 64 years. Provides patient care under the direction of the Registered Nurse and within the auspices of the licensure Qualified individuals must have the ability with or without reasonable accommodation to perform the following duties: Provides clinical advice by phone electronic correspondence based on clinical judgments and or triage protocols in accordance with regulatory requirements and standards of practice. Consults with provider as necessary. Provides patient specific education including but not limited to preventive care, chronic disease management, acute diagnosis, medications and other areas pertinent to patient outcomes. Prepares patient for visit by using appropriate rooming standards tool. Gathers information from patient and directs to the RN or Provider when necessary. Administers injections and medications as prescribed by the provider and in accordance with department protocol, regulatory requirements and standards of practice. Prepares and assists providers with various procedures, minor surgeries and other exams as designated by the provider. Performs Point of Care Testing, including but not limited to hearing, flu, urine and strep tests. Collects blood or other laboratory specimens and prepares them for further testing. Authorizes prescription refills as instructed by the provider. Assists with test tracking and monitoring overdue results. Removes sutures and changes dressings on wounds as necessary. Participates in daily huddles with the provider. Serves as a patient experience advocate by being a positive influence at all times when interacting with patients, visitors and colleagues. Communicates with all patients, visitors and colleagues in a professional manner while keeping customer service a top priority. Handles all patient information in a confidential manner according to Lancaster General Health privacy and confidentiality policies and procedures. Other duties as assigned. Minimum Required Qualifications: Completion of an LPN educational program Current licensure as a Licensed Practical Nurse LPN, issued by the PA State Board of Nursing American Heart Association CPR certification Preferred Qualifications: One 1 to Two 2 years of nursing experience in a primary or specialty care physician’s office.
