Resposibilties The CNA maintains a "homelike environment for the residents, protects and promotes Resident Rights and assists the person to maintain independence and control to the greatest extent possible. Provides direct care to residents to improve or maintain the resident’s abilities Follows all facility policies and procedures including those related to infection control. Records accurate, legible information about resident care and condition in appropriate sections of residents’ records. Communicates with Unit Charge Nurse about assigned residents regularly throughout shift. Monitors physical environment for safety or fire hazards and corrects or reports potential problems Assesses, implements, and evaluates care needed by assigned patients under the direction of the licensed physician. The RN is knowledgeable about principles, theories, current methods and techniques of nursing practice. Using the nursing process, the RN collaborates with the patient, patient's family member(s)/significant others, professional peers and other members of the health care team to individualize patient care. Directs nursing support staff in the provision of patient care according to the standards of nursing practice, administrative and departmental policies and procedures. Professional education, training or additional certifications may be required by specialty area. MDS Coordinator Location: Hamilton, NJ Facility: 55-Bed Skilled Nursing Facility Schedule: Full-Time, Monday – Friday Accela Healthcare is seeking an experienced and detail-oriented MDS Coordinator to join our Hamilton team. In this role, you will oversee and coordinate the completion of the Minimum Data Set (MDS) assessments to ensure accuracy, timeliness, and compliance with federal and state guidelines. As a vital member of our clinical leadership team, you’ll help support quality resident care and accurate reimbursement for our 55-bed facility. Key Responsibilities Coordinate, complete, and submit MDS assessments in compliance with state and federal regulations. Ensure accuracy of resident assessments, care plans, and supporting documentation. Collaborate with interdisciplinary team members to gather information and develop individualized care plans. Monitor schedules to ensure timely completion of assessments. Educate and support nursing staff on MDS processes, PDPM requirements, and documentation practices. Participate in quality improvement initiatives and audits as needed. Qualifications Registered Nurse (RN) preferred; Licensed Practical Nurse (LPN) with strong MDS experience considered. Prior experience as an MDS Coordinator in a skilled nursing facility strongly preferred. Knowledge of PDPM and current federal/state regulations. Strong organizational, communication, and critical thinking skills. Ability to work independently and collaboratively with an interdisciplinary team. Why Join Accela Healthcare?
