AREAS OF RESPONSIBILITY: Admits patients to service. Assesses patient’s environment; physical, mental and social status and eligibility for hospice services in accordance with hospice admission requirements. Assures completion of all documentation including the plan of care Assures that all applicable services are ordered, explained, and offered. Obtains physician orders for services, equipment, medication as needed. Assesses need for and coordinates services of all other IDG member, including Physician, Social Worker, Home Health Aide, Dietician, Volunteer, and Spiritual Care. Documents observations, clinical findings, problems, skilled interventions, goals and plans in a timely manner (including documenting appropriately on Point of Care/laptop device). Makes skilled nursing revisits to established patients as directed by Patient Care Manager. Provides and documents skilled nursing care (includes skilled observation of the patient’s condition, skilled care and procedures and teaching of the patient and/or family), on all intermittent visits and/or shifts. Provides supervisory visits for the Home Health Aides as directed and as they spontaneously occur. Demonstrates all skills of a registered nurse, including those skills required by the agency through competency testing. Maintains and updates own level of knowledge and skills required to provide quality hospice nursing care. Participates in QA / PI activities and in-service education related to areas of practice as appropriate. Assists in development of hospice protocols, procedures and policies as requested. Performs other duties as assigned consistent with skills and training and the mission and goals of Pathways. QUALIFICATIONS: California RN license required: PHN preferred. Minimum of one-year experience as a professional nurse within the last three years or a baccalaureate degree in nursing from a program accredited by the National League for Nursing. Must be highly motivated, self directed, flexible, and able to work well and participate in a multidisciplinary team setting. Current California driver’s license and automobile insurance. Job Summary: Selects and administers intravenous, spinal and other anesthetics prior to and during surgical or medical procedures, as a member of the anesthesia care team and in accordance with Kaiser Permanente policies and procedures. Monitors patient reactions and vital signs during and after anesthesia and takes remedial action when warranted. Submits postoperative report on patient condition, and maintains records of anesthetics administered. Performs procedures outside of the OR, in accordance with KP policies and procedures, established competency, and MD order. Essential Responsibilities: Conducts and/or reviews anesthetic evaluation of patients prior to surgical proceedings, deliveries or other pertinent medical procedures in hospital setting; consults with anesthesiologists and/or surgeons. Evaluates patients to identify apparent difficulties with airway control, etc. Ensures that all pre-anesthesia evaluations are documented thoroughly according to policies and standards. Explains tests, procedures and disease processes to patients and their family as indicated; explains the major anesthetic risks to patients as requested and documents that the risks are explained to patients. Develops the care regimen of assigned patients; reviews the care regimen with the supervising physician and gains concurrence; implements the anesthesia plan; starts IV solution, administers selected anesthetic, choice of anesthetic consistent with patient safety, surgical need, possible post operative complications, and patients history and conditions. and evaluates and manages the patient, according to departmental policy. Participates with post anesthesia care unit personnel in a patient safety oriented interface when patients are transferred from the surgery suite. Monitors patients vital signs, such as blood pressure and observes any significant physical changes; informs anesthesiologist and/or surgeon of patients condition and takes any necessary remedial action such as infant resuscitation when necessary. Mixes solutions to required concentrations, such as pentothal, succinylcholine, vaso-active drugs etc. Keeps current accurate and complete record of procedures performed and patients conditions immediately prior to the administration of anesthesia according to established standards. Performs procedures done outside of OR, such as intubation, arterial line placement, etc., in accordance with policies and procedures, competencies, and physician order. Charts details of procedures done on progress notes. Kaiser Permanente conducts compensation reviews of positions on a routine basis. The RN Case Manager, when assigned by the Clinical Manager/DPCS, assumes responsibility to coordinate patient care for an assigned case load. The Admission RN may act as Case Manager when assigned by Clinical Manager/DPCS and assumes responsibility to coordinate patient care for assigned caseload. The RN Case Manager may act as an Admission Nurse when assigned by Clinical Manager/DPCS and assumes responsibility to coordinate with intake, program staff and management to complete a patient initial admission/assessment with plan of care. Communication: Completes, maintains, and submits accurate and relevant clinical notes regarding patient’s condition and care given. Records pain/symptom management changes/outcomes as appropriate. Communicates with the physician regarding the patient’s needs and reports changes in the patient’s condition; obtains/receives physicians’ orders as required. Communicates with community health related persons to coordinate the care plan. Teaches the patient and family/caregiver self-care techniques as appropriate. Provides medication, diet and other instructions as ordered by the physician and recognizes and utilizes opportunities for health counseling with patients and families/caregivers. Works in concert with the interdisciplinary group. Provides and maintains a safe environment for the patient. Assists the patient and family/caregiver and other team members in providing continuity of care. Works in cooperation with the family/caregiver and hospice interdisciplinary group to meet the emotional needs of the patient and family/caregiver. Attends interdisciplinary group meetings. Additional Duties: Participates in on-call duties as defined by the on-call policy. Ensures that arrangements for equipment and other necessary items and services are available. Supervises ancillary personnel and delegates responsibilities when required. Assumes responsibility for personal growth and development and maintains and upgrades professional knowledge and practice skills through attendance and participation in continuing education and Inservice classes. Fulfills the obligation of requested and/or accepted case assignments. Actively participates in quality assessment performance improvement teams and activities. Current licensure in state of practice and current CPR certification. Must possess a valid California Driver’s License and can show proof of valid auto insurance. California only: May have an RN license and practiced as an RN for a minimum of one year with prior LVN experience in public health, home care, Hospice, Hospital or Skilled nursing facility. All States: Registered nurses shall have a minimum of (a) one (1) year of experience as a professional nurse within the last three (3) years OR (b) have a baccalaureate degree in nursing from a program accredited by the National League for Nursing and a current RN license. A minimum of two years’ experience, at least one of which is in public health, home care, or hospice nursing is preferred. Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working order. Management experience not required. Responsible for supervising hospice aides. Self-directed and able to work with minimal supervision. Excellent observation, verbal and written communication skills, problem solving skills, basic math skills; nursing skills per competency checklist. Prolonged or considerable walking or standing. Able to lift, position and/or transfer patients. Able to lift supplies and equipment. Considerable reaching, stop bending, kneeling and/or crouching.
