Full Job Description Purpose To provide supervision, coordination, and evaluation of the day-to-day nursing activities in accordance with current federal, state, and local standards, guidelines, and regulations that govern the facility on a 24 hours per day, 7 days per week basis on an assigned skilled nursing unit. Essential Functions Assumes 24-hour oversight for resident care on assigned unit. Ensures that all nursing personnel assigned to you comply with the written policies and procedures established by this facility. Periodically reviews the department’s policies, procedure manuals, job descriptions, etc. Makes recommendations for revisions. Periodically reviews the resident’s written care plan. Participates in updating of the resident’s written care plan as required. Completes and reviews accident/incident reports and follow up as necessary to QA Director and DON/ADON. Reports and investigates all allegations of resident abuse (including physical, verbal, mental, sexual, involuntary seclusion, neglect, and misappropriation of property.) Completes daily, weekly, and monthly audits and submits to ADON/DON/QA as required. Maintains the Daily Census Report and submits to the Business Office as required. Makes rounds on unit to ensure resident care needs and environmental standards are met. Unit Manager is responsible for investigating and managing any problem areas on their unit and reporting outcomes to the ADON/DON. Actively participates in committees/programs as directed by the ADON/DON. Actively participates in the Quality Assurance process, including attendance at Quality Assurance meetings and submission of required reports each month. Ensures changes in resident condition are communicated to the physician, family or responsible party. Meets with residents, and/or family members, as necessary. Attends and participates in annual OSHA and CDC in-service training programs for hazard communication, TB management, and blood-borne pathogens standard. Ensures that all personnel performing tasks that involve potential exposure to blood/body fluids participate in appropriate in-service training prior to performing such tasks. Participates in the development, implementation, and maintenance of the infection control program for monitoring communicable and/or infectious diseases. Maintains a written record of resident grievances and indicates actions taken to resolve and the current status of the grievance. Participates in facility surveys (inspections) made by authorized government agencies. Holds staff meetings for all 3 shifts as least once per month. Acts as a resource for nursing staff. Works collaboratively with ADON/DON and Staff Development to identify and provide orientation and continuing education for unit staff members. Provides direct nursing care as necessary. Performs other related duties as assigned. Work Schedule As assigned Working Conditions Is subject to frequent interruptions. The noise level in the work environment is usually moderate. Is involved with residents, personnel, visitors, etc., under all conditions and circumstances. Is subject to hostile and emotionally upset residents, family members, etc. Communicates with the medical staff, nursing staff, and other department supervisors. Works beyond normal duty hours, on weekends, and in other positions temporarily, when necessary. May be exposed to bodily fluids, infectious waste, diseases, conditions, etc., including the AIDS and hepatitis B viruses. Physical Requirements While performing the duties of this job, the employee is occasionally required to stand; walk; use hands to finger, handle, or feel; implements or office equipment; reach with hands and arms; stoop, kneel, crouch, or crawl and talk or hear. Must be able to speak and write the English language in an understandable manner. Collect information through observation, listening, touching, and smelling. Must have auditory ability sufficient to monitor and assess Job requires regular sitting, standing, walking, stooping, bending, kneeling, crouching, reaching, pushing, pulling, twisting, writing, and carrying charts, food, trays, etc. Must be able to move intermittently throughout the work day. Specific vision abilities required by this job include close, distance and color vision and the ability to adjust focus. Employee may be required to move chairs, beds, and medical equipment, move patients in bed (with or without assistance). Must be able to cope with the mental and emotional stress of the position. Must function independently, have flexibility, personal integrity, and the ability to work effectively with the residents, personnel, and support agencies. Must be in good general health and demonstrate emotional stability. Must be able to relate to and work with ill, disabled, elderly, emotionally upset, and at times hostile people within the facility. May be required to lift, push, pull, and move equipment, supplies, etc., in excess of fifty (50) pounds. Must be able to assist in the evacuation of residents. Safety, Sanitation and Infection Controls Assure the assigned work areas are maintained in a clean, safe and sanitary manner. Assure that established infection control and universal precautions practices are observed. Report all hazardous conditions or equipment to your supervisor. Follow established safety policies and procedures. Dispose of refuse daily in accordance with established sanitation procedures. Keep work/assignment areas free of hazardous objects, unnecessary equipment, supplies, etc. Report any accident/incident to your supervisor no matter how minor. Qualifications Must have, at a minimum, a Nursing Degree from an accredited college or university, or be a graduate of an approved RN or LPN program. Must possess a current, unencumbered, active license to practice as an RN or LPN in this state. Must be able to read, write, speak, and understand the English language. Must possess the ability to make independent decisions when circumstances warrant. Must possess the ability to deal tactfully with personnel, residents, family members, visitors, personnel, and the general public. Must be knowledgeable of nursing and medical practices and procedures, as well as laws, regulations, and guidelines that pertain to long-term care. Must possess leadership and supervisory ability and the willingness to work harmoniously with and supervise other personnel. Supervisory experience preferred, but not required. Must be willing to seek out new methods and principles and be willing to incorporate them into existing nursing practices. Must be able to relate information concerning a resident’s condition. JOB DESCRIPTION SUMMARY The hospice aide works under the supervision of a registered nurse and performs various tasks as assigned in accordance with the patient’s plan of care to meet the patient's personal needs and to promote comfort. The hospice aide is responsible for reporting and documenting observations and care performed. The hospice aide will be assigned in a manner that promotes quality, continuity and safety of a patient's care. ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES Responsibilities of the hospice aide include, but are not limited to, the following: 1. Performing personal care needs after given a patient's assignment and attending to his/her requests promptly. If unable to perform a certain task, report to the Case Manager immediately. 2. Providing patient and family with positive communication techniques, within realm of ethical and respectful care, including confidentiality. 3. Meeting safety needs of patient and using equipment safely and properly (foot stools, side rails, O2 etc.) 4. Giving personal care including baths, back rubs, oral hygiene, shampoos and changing bed linen as often as assigned. 5. Assisting in dressing and undressing patients as assigned. 6. Assisting in feeding the patient as assigned. Hospice Aides may not provide patients with homemade food items or baked goods that they themselves prepared 7. Providing proper care and observation of patient's skin to prevent breakdown of tissue over bony prominence. 8. Reporting any complaint of pain or other symptoms or changes in the patient's condition to the RN Case Manager. Also aware of the caregiver or other individuals living with the patient and interpersonal issues. 9. Assisting in ambulation and exercise as instructed by the hospice nurse or therapist. 10. Provide normal range of motion and transfers/positioning as directed. 11. Assistance with self-administered medications as allowed by state regulations, such as “reminders.” 12. Offering and assisting with bedpans and urinals. Providing assistance as assigned with light laundry needs, emptying waste baskets and other light homemaker tasks. 13. Performing range of motion and other simple procedures as ordered with appropriate completed/demonstrated skills competency. 14. Providing respite for patient's/family/caregiver when on-site, as appropriate. 15. Keeping patient's living area clean and orderly, as assigned. 16. Adhering to the organization's documentation and care procedures and standards of personal and professional conduct. 17. Participates in quality assessment performance improvement teams and activities. 18. Respectful of patient and family/caregiver environment and patient's personal needs. 19. As a mandatory reporter, reports failure to comply with the requirements of the Dept. of Health as required within 14 calendar days, using Dept. of Health forms. 20. Reports suspected abandonment, abuse, financial exploitation, or neglect of a person in violation of the department of social and health services and the proper law enforcement agency.
