PRIMARY TASKS AND RESPONSIBILITIES : · Assess patients' physical and psychosocial status through interviews, health histories, physical examinations, and diagnostic studies. · Write prescriptions for medication and blood products based on laboratory results, routine diagnostic and follow-up studies, therapeutic measures, and care post-discharge in accordance with written practice protocols. · Based on findings, determine patients' eligibility for entry into clinical trials. · Interpret and evaluate findings of studies/tests. · Relay appropriate information regarding patient care to the collaborating physician. · Administer therapeutic measures / obtain specimens as prescribed. · Dictate follow-up letters to referring physicians with summaries of treatment, response and plan. · Dictate or writes patient history, admissions, progress notes, and discharge notes in a timely manner. · Assist in the management of family dynamics and coping mechanisms during acute and chronic phases of patients’ care. · Conduct or participate in daily multi-disciplinary rounds for assigned patients to ensure patients are receiving appropriate and quality care. · Provide on-call coverage and perform hospital rounds as needed. · Triage patients to determine the urgency of physician evaluation. · Document and report care in an accurate, timely manner using appropriate forms and records. · Demonstrate a willingness to learn and to remain flexible in the changing healthcare environment. · Assist staff in times of crisis or emergency. · Meet with the public outside the institution and efficiently market and promote cancer treatment. · Determine work priorities, assign work and insure proper completion of work assignments. · Comply with all federal and state laws and regulations pertaining to patient privacy, patient’s rights, personnel law, safety, labor, and employment law. Job Description Part Time/Every other weekend/7AM-3PM or 3PM-11PM, Saturday & Sunday Responsible for supervision of self-administered medication for the Memory Care Department in compliance with Federal & State regulations and Senior Lifestyle Corporation guidelines as directed by the LPN/LVN/RN and/or the Director of Health & Wellness. Count and sign on approved form, all narcotics with approved personnel before accepting medication keys. Contact a supervisor immediately with discrepancies. Order all necessary medication and treatments from appropriate pharmacy or notify responsible party when supply is no less than 7 days. Notify supervisor when supply is less than 2 days and medicine hasn’t been received. Responsible for receiving and verifying all medicine received is accurate according to pharmacy label and MAR. Notifies pharmacy and DH&W of any discrepancies. Responsible for proper storage of all medications and ensuring all lotions, ointments and liquids stored separately from oral medications. Responsible for dating all appropriate vials or bottles at time of initial opening and following pharmacy protocol on length of time they can be used. Responsible for weekly inspections of all medications to ensure all expired medications are removed. Responsible for verifying all current MARs are compared to new MARs at change over time and notifying the supervisor of any discrepancies. Responsible for removing all discontinued medication from med cart and giving it to the supervisor. Provide personal care to residents to include, but not limited to, bathing, skin care, oral hygiene, grooming, dressing, incontinence care, and eating, etc. Provide general nursing care to include, but not limited to, vital signs, temperatures, blood pressure, and weights. Document appropriate information in the daily log as required and notify supervisor of all concerns. Answer calls and assist residents with needs as assigned. Responsible for daily cleaning of all medicine carts at end of shift and restocking as needed. Responsible for weekly deep cleaning of all med carts. Attend and participate in staff training and in-services. Must maintain accurate records to include preparation of all medical charts for new admissions and readmissions and ensures medical record is complete within forty-eight (48) hours as Policy and Procedure (P&P) and regulatory guidelines. ESSENTIAL DUTIES AND RESPONSIBILITIES – Other duties may be assigned. · Triage patients by injury or illness. · Fulfills patient care responsibilities as assigned, which may include checking schedules and organizing patient flow · Escorting patients to exam/procedure rooms. Adhere to infection control safety guidelines, maintaining equipment, instruments, and stock supplies. · Utilizes electronic referrals process to complete requested physician referrals to NCHMD specialists and ancillary services. · Performs diagnostic x-ray with minimal exposure to self or patients. Correctly identifies patient prior to exam, introduce self and receive signature for ABN if required. Keep radiology area stocked.
