Assists with patient examinations, procedures, diagnostic studies, treatment, and dressing changes. Performs general administrative duties in accordance with JCAHO, CLIA, NCQA and government standards, as well as company policies and procedures. Key Responsibilities Performs visit chart preparation activities based on the daily scheduled appointments including diagnostic test results, emergency/urgent care reports, discharge summaries and consult/PCP reports are located in the medical record and accessible to the provider. Performs rooming activities for provider visits according to the documented rooming criteria including chief complaint, allergies, vital signs, medication list review, tobacco history, LMP, health maintenance overrides and documentation in a timely and accurate manner. Obtains and documents age and/or condition-specific measurements accurately including vital signs, height, weight, head circumference, peak flow, and pulse oximetry. Conducts and documents hearing, vision, respiratory and ECG testing. Performs and documents specific screening based on condition/need including depression and asthma. Obtains, documents and processes specimens using the appropriate collection process and equipment. Performs and documents point of care testing accurately. Performs venipuncture procedures (as needed) and processes specimens as outlined in policies and procedures. Administers medications and immunizations including oral, nasal, injectable, rectal and topical routes as appropriate. Processes medication prescription refills per guidelines using telephonic, electronic or print methods. Handles prior authorization processes for medications and/or procedures and documents appropriately. Manages clinical supplies and medication sample areas to ensure that stock is current, stored appropriately, labeled and par levels maintained. Education/Experience/Licensure Education: High School Diploma or GED required. Graduate from an accredited Medical Assistant program required. Experience: One (1) to three (3) years medical assistant experience preferred. Certification/Licensure: Medical Assistant Certification/ABR-OE strongly recommended. Updates and maintains patient assignment board, Census Change Sheets, and all other patient lists. Supports effective and efficient patient care processes through timely, accurate transcription and entry of orders/data. Maintains Unit Secretary task list to ensure patient care which are verified by the registered nurse, to appropriate patient care documents. Assembles patient charts for all admissions to the unit to include all appropriate documents Prepares patient charts for patient transfers by copying and organizing required documents according to specific payer criteria. Disassembles and reorganizes charts upon discharge before forwarding to medical records. Coordinates completion of patient death certificates by contacting appropriate parties. Supports the smooth functioning of the unit by performing a variety of clerical and coordinating activities as designated by the unit. Updates and maintains the daily communication device list for the unit; ensures that all RNs receive communication device at the beginning of the shift and return them at the end of the shift. Radiation Therapy Schedules, tracks and assures timeliness of diagnostic testing. Obtaining insurance authorization for diagnostic testing. Schedules all follow up clinic appointments. Reviews patient’s charts to ensure all necessary information is obtained prior to patient visit. Obtains an updated medication list for all follow-up patients. Coordinates needed services/referrals for follow up patients. Tracks patients missed appointments, and notification to patient to reschedule and sending a certified notice for repeated missed appointments, per protocol. Assists physicians with patient physical exams/procedures/preparation of needed equipment. Ensures all exam rooms are adequately stocked and cleaned. Checking for and replacement of any expired items. Responsible for lined inventory and distribution within the department. Coordinates weekly treatment visits for physicians and RNs. Orders all departmental needed clinical supplies in the Lawson system. Tracks supply of bulk stores and MDS items and makes recommendations to adjust par levels. Telesitter Maintains constant visual observation of the patients receiving remote visual monitoring. Participates in the hand-off, during shift change or when admitting new patients to the system, of pertinent information/behavior about patients being monitored. Ensures another telesitter in place before leaving monitoring station. Verbally redirects patients when necessary. Provides patients with explanations as necessary, but does not counsel or provide opinions. Who You Are: Current Nursing Assistant License in the State of New Hampshire required. BLS authorization preferred or obtained within 90 days of date of hire. High school diploma or equivalent preferred. Whittier Rehabilitation Hospital - Transitional Care Unit/SNF - Haverhill MA Unit Manager - RN 20 Bed Transitional housed within our Rehabilitation Hospital Regular Schedule will be M-F days (40 hours), with coverage as needed on weekends and off-shifts. We are seeking a Nurse Manager to oversee and lead our Transitional Care Unit housed within our beautiful Rehabilitation Hospital on Ward Hill in Haverhill Ma.
