Assesses assigned case load of home health care patients and families to identify the physical, psychosocial, and environmental needs of patients as evidenced by documentation, clinical records, Interdisciplinary Team reports, after hours reports, and on-site evaluations. Assumes primary responsibility for caseload, including assessing, planning, coordinating, implementing, and evaluating the plan of treatment. Assesses patient needs and obtains data on physical, psychological, social, and spiritual factors that may influence patient/family/caregiver health status and incorporate that data into the plan of care. Makes the initial evaluation and re-evaluates the patient’s nursing needs during each visit. Makes follow up visits according to patient, family, and facility needs. Communicates significant findings, problems, and changes in health condition, environment, or unsafe facility conditions to the Clinical Supervisor, physician, facility, and/or other personnel involved with patient care. Revises the plan of care in response to identified patient care issues and notifies the Clinical Supervisor, physician, facility, and other team members. What is Required? Current, unrestricted state RN licensee 1+ year experience in a clinical care setting as a nurse Able and willing to travel within branch/office coverage area. Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment Must have a dependable vehicle, valid driver’s license, and current auto insurance in accordance with state laws. The program begins in May after spring semester ends and concludes before the start of the fall semester. Participants complete a two-week orientation. This temporary PRN role includes a competitive hourly wage, requires 36 hours per week across three 12-hour shifts, and may include nights and weekends . Clinical Experience Students work under the direct supervision of registered nurse preceptors in various inpatient clinical units. Orientation and ongoing clinical support help students develop real-world nursing skills, confidence, and readiness for practice. Compensation This is a temporary, hourly-paid position requiring a 36-hour work week. The position is not eligible for benefits. Ensure that all reports of apparent or suspected violations of rights within Fieldstone Center are investigated in accordance with section 330.1778 of the Michigan Mental Health Code. The policy requires that the rights office must conduct investigations in a manner that does not violate employee rights. Those reports that do not warrant investigation are intervened upon and recorded in accordance with subdivision (d) of the Michigan Mental Health Code. Semiannually provide summary complaint data consistent with the annual report required by the Code, together with a summary of remedial action taken on substantiated complaints by category, to the State of Michigan and to the Recipient Rights Advisory Committee. D) Training received by associates of the Office of Recipient Rights. E) Training provided by the Office of Recipient Rights.
