The Care Giver/Med Tech role includes providing hands on care, physical and emotional support to each resident while maintaining a safe and comfortable home like environment. Responsibilities: Care Giver: Responsible for a designated group of residents during the shift; knows where their residents are and physically checks on them throughout the shift. Observes, reports and documents symptoms and conditions of residents for changes in condition such as skin, behavior, alertness, weight, dietary and participation in activities. Responds to security system and resident call bells promptly. Notifies supervisor and/or Health Care Coordinator if a resident has increased care needs. Assists with continence management and disposes of all continence products properly to ensure sanitation of resident suite and community restrooms. Serves and collects food trays and assists as needed; reports refusal of meals or loss of appetite. Maintains cleanliness of resident’s room and work areas. Practices good standard care precautions of cleanliness, hygiene and health. Helps residents maintain independence, promotes dignity and physical safety of each resident. Actively participates/leads and assists residents with activities of daily living (i.e. bathing, dressing, toileting, grooming, ambulation, transferring, eating) as instructed. Engages residents in life skills and other life enrichment activities. Greets and assists all internal and external customers, guests, family members, residents, vendors and team members. Does resident laundry as assigned and needed. Medication Tech duties: Reviews service plan to learn pertinent information about residents. Assists/observes medications and treatments for each resident using the medication observation record and the Six Rights of Medication Pass. Documents and initials form as medications are given including appropriate documentation for refusal or missed doses. Maintains confidentiality of all resident information including resident medication. Reports all resident concerns made while assisting resident with the medication to the Nurse and/or Health Care Coordinator (HCC). Restocks medication cart after all medication passes. Assists in checking medication regardless of packaging system. Counts all narcotics with another Medication Care Manager or Nurse each shift. Maintains and cleans the Medication Room, med charts, treatment carts for neatness cleanliness, availability of medications and expired medications. Follows re-fill process for medications. Participates in the development of the Service Plan and monthly updates. Takes and records temperature, blood pressure, weight, pulse and respiration rates. Reports occupational exposures to blood, body fluids, infectious materials, sharp sticks and hazardous chemicals immediately. Other duties as assigned by the Supervisor. Qualifications: Certified Nurse’s Aid certification preferred. High School diploma/GED Must be 18 years of age. CPR Certification preferred First Aid Certification preferred Previous experience working with seniors preferred. In this role you will be responsible for the safe administration of all prescribed medications and treatments as listed on the Medication Administration Record (MAR) and Treatment Administration Record (TAR), provides personal are and general nursing care to residents as needed or assigned, ensures delivery of care and services to all residents in the community to include Activities of Daily Living (ADLs), room cleanliness, scheduled activities and dining services and individual service plan need of each assigned resident. Job Description Responsible for supervision of self-administered medication for the Assisted Living Department in compliance with Federal & State regulations and Senior Lifestyle Corporation guidelines as directed by the LPN/LVN/RN and/or the Assisted Living Director/Health and Wellness Director. Count and sign on approved form, all narcotics with approved personnel before accepting medication keys. Contact a supervisor immediately with discrepancies. Order all necessary medication and treatments from appropriate pharmacy or notify responsible party when supply is no less than 7 days. Notify supervisor when supply is less than 2 days and medicine hasn’t been received. Responsible for receiving and verifying all medicine received is accurate according to pharmacy label and MAR. Notifies pharmacy and DH&W of any discrepancies. Responsible for proper storage of all medications and ensuring all lotions, ointments and liquids stored separately from oral medications. Responsible for dating all appropriate vials or bottles at time of initial opening and following pharmacy protocol on length of time they can be used. Responsible for weekly inspections of all medications to ensure all expired medications are removed. Responsible for verifying all current MARs are compared to new MARs at change over time and notifying the supervisor of any discrepancies. Responsible for removing all discontinued medication from med cart and giving it to the supervisor. Provide personal care to residents to include, but not limited to, bathing, skin care, oral hygiene, grooming, dressing, incontinence care, and eating, etc. Provide general nursing care to include, but not limited to, vital signs, temperatures, blood pressure, and weights. Document appropriate information in the daily log as required and notify supervisor of all concerns. Answer calls and assist residents with needs as assigned. Responsible for daily cleaning of all medicine carts at end of shift and restocking as needed. Responsible for weekly deep cleaning of all med carts. Attend and participate in staff training and in-services. Must maintain accurate records to include preparation of all medical charts for new admissions and readmissions and ensures medical record is complete within forty-eight (48) hours as Policy and Procedure (P&P) and regulatory guidelines. Qualifications Level of Formal Education: A High School Diploma or General Education Degree; or one to three months related experience and/or training; or equivalent combination of education and experience Years of Experience: 1+ months of experience Type of Experience: Senior Living preferred Special Certifications: Current Medication Technician certification depending on state regulations Language Skills: Acceptable proficiency in English to perform job duties, communicate with supervisor and other employees, and respond and communicate to emergency situations. Responsibilities Key Responsibilities: Work under the supervision of a licensed vocational nurse or a registered professional nurse to perform the following duties: After authorization by the facility’s licensed nurse or the resident’s treating physician, administers PRN medications. Organizes assigned medication duties to pour, pass, and document all routine prescribed medications and to give PRN medication only upon the instruction of the licensed nurse. Must document all PRN’s according to State Standards in all areas of the chart. Observe and report to the facility’s charge licensed nurse reactions and side effects of medication commonly administered to nursing facility’s residents. Take and record vital signs prior to administration of medications which could affect or change the vital signs. Administer and document regularly prescribed medications which the medication aide is permitted to administer only after personally preparing (setting up) of those medications to be administered. Document the medications given in the resident’s clinical record. Administer oxygen per nasal channel or a non-sealing face mask only in an emergency. Immediately after the emergency, the medication aide shall verbally notify the licensed nurse on duty or on call and appropriately document the activity and notification. At discretion of charge nurse, count narcotics, give all routine prescribed narcotics, and document same on individual sheet and medication sheet. Qualifications Must have successfully completed state approved school of medication administration. Must carry current acknowledgment card or certification issued by state agency.
