Job Description The Lead Case Manager is responsible for assessing the strengths and needs of homeless clients, developing care plans for clients within their service area, and overseeing client care within their assigned program. The Lead Case Manager will work closely with the Program Manager to ensure contract compliance and outcomes. The pay range for this position is $26.00 - $28.00 per hour, depending on experience. The schedule for this position is Monday - Friday, 9:00am to 5:30pm. Responsibilities Client Care Oversight Develop care plans for clients within assigned programs. Oversee the care plans in partnership with the Program Manager by conducting monthly meetings with care teams Oversee client assessments to evaluate client’s need, develop treatment plan for client, monitor progress, provide assistance and referrals, set-up and follow-up with weekly goals for case managers to work on with clients.
