Description
As an Extended Care (ECU) Nurse, you will provide high quality, compassionate care. Your patients are those recovering from a stroke, orthopedic surgery, respiratory or cardiac illness, and other conditions.
The (ECU) serves as a transitional unit where a patient can improve their health after a procedure before returning home. Community First’s skilled interdisciplinary extended care team helps each patient and their family achieve their highest level of functioning and safety before being discharged.
This is a two or three Days a week - 8 or 12 hour shifts, This opportunity comes with a Hire On Bonus.
ESSENTIAL DUTIES AND RESPONSIBILITIES
1. Asses each patient's biophysical, psychosocial, spiritual, cultural, self-care, discharge planning.
2. Utilizes the caring behaviors of empathy, active listening and cultural sensitivity and supports patient dignity.
3. Develops and prioritizes an outcome directed plan of care to assigned patients and changes as necessary.
4. Performs clinical and technical aspects of care in accordance with procedures, standards of practice.
5. Evaluates the effectiveness of nursing interventions and modify and develop patient outcomes.
6. Reviews the plan of care and consults with the physician and care providers to adjust optimum outcome.
7. Documents care per documentation standards.
8. Provides patient/family education.
9. Serves as a mentor/preceptors for colleagues and other staff within or outside of areas of responsibility.

Education and/or Experience
- RN Liiinois license or pending
- Ability to follow instructions and record information accurately.
- Able to communicate effectively using written and verbal English.
- Good interpersonal skills.
- EMR/EPIC experience preferred
- BLS/CPR certification required.
