OverviewWorks independently to develop and implement a referral growth plan to meet agency volume targets. Demonstrates knowledge and expertise of all home care services available in Riverside geographic region. Educates community on services through outreach events. Builds relationships with physicians, practices, care coordinators and other referral sources to promote the benefits and quality outcomes of services. Serves as clinical liaison with physicians and care managers to include meeting with patients and families to facilitate and evaluate readiness for transition to home. Collaborates with intake coordinators on patient referrals as needed. Provides feedback and participates in process improvement initiatives that enhance quality and customer service outcomes to meet referral targets.
What you will do- Participates in hospital IPOC rounds and other care/discharge meetings to identify patients who meet criteria and would benefit from home health, in-home private care, palliative care or hospice services. Builds relationships with physicians, care managers and other healthcare professionals and provides education about the benefits and quality outcomes of services that best meet the needs of their patients. Coordinates home care services to facilitate transitions of care and meets with patients and families to explain the benefits and what to expect from their home care service.
- Communicates feedback from referral sources, patients, families and other customers regarding quality of care and/or service experiences to agency leadership in assigned territories. Participates in process improvement initiatives to address areas of opportunity. Provides effective and timely follow-up to resolve issues and handle complaints. Collaborates with intake coordinators as needed to complete new patient referrals.
- Responsible for developing growth plan and implementing activities to drive referrals to meet/exceed admission targets for home health, in-home private care, palliative care and hospice services. Effectively identifies referral sources with the greatest potential and successfully executes plan to increase and maintain referrals from those referral sources. Provides high quality customer support and service to facilitate relationship building and trust.
- Facilitates goals of care conversations with patients and families and completion of advance directives.
- Monitors sales targets and analyzes reports for their assigned territory.
- Participates in community events designed to promote education and understanding of the benefits of home care services and how to access care. Events may include community health/vendor fairs, senior conferences/forums, lunch and learns, service club presentations and other events.
QualificationsEducation- Bachelors Degree, Nursing (Preferred)
- Associates Degree, Nursing (Preferred)
- Program Graduate, Nursing Diploma (Required) and
- High School Diploma or GED, Only for Nursing Diploma Graduates (Required)
Experience- 1 year RN Experience Home Care (Preferred)
Licenses and Certifications- Registered Nurse (RN) - Virginia Department of Health Professions (VDHP) or licensed in a compact state (Required)
- CPR/BLS Certification - American Heart Association/American Red Cross/American Safety and Health Institute (AHA/ARC) within 30 Days(Required)
- Drives personal vehicle for RHS business 25% or more of the time to perform essential functions of the job
- Valid Drivers License Required
