As a part of the Telecare Family, La Paz is a 148 bed Gero-Psychiatric center which serves adults 55 and older.

Shifts Available:

Full Time; AM 8:00 am - 4:30 pm; Monday - Friday

Expected starting wage range is $102,460.28 - $126,595.30. Telecare applies geographic differentials to its pay ranges. The pay range assigned to this role will be based on the geographic location from which the role is performed. Starting pay is commensurate with relevant experience above the minimum requirements.

POSITION SUMMARY
The RN MDS Coordinator coordinates the treatment care plan process with the Interdisciplinary Treatment Care Plan Team. This involves coordination of all care provided to members served with a focus on maintaining the quality of services rendered.

ESSENTIAL FUNCTIONS
• Demonstrates the Telecare mission, purpose, values and beliefs in everyday language and contact with the internal and external stakeholders
• Plans, organizes, and directs Interdisciplinary Treatment Team Meeting
• Promotes and maintains harmonious and effective relationships and communications within the department and other departments
• Makes use of management practices that include empowerment of staff, the provision of clear and concise expectations regarding duties assigned employees, frequent feedback focusing on both positive and problematic aspects of work performance and other management practices that are consistent with Continuous Quality Improvement
• Responsible for completion of MDS utilizing Interdisciplinary Treatment Plan processes and related documentation including but not limited to Resident Assessment Protocols (RAPS) care plan
• Provides safe, effective and efficient implementation of direct care in accordance with established policies, procedures and standards of care
• Establishes and maintains a therapeutic relationship with members served
• Reviews and maintains the amount, accuracy and quality of documentation included in the medical records
• Participates as a team member and provides input via reporting observations, concerns and asking appropriate questions
• Reviews and implements members served treatment plan in accordance with MDS
• Actively participates in multidisciplinary team meetings and treatment planning meetings
• Demonstrates knowledge of management of assaultive behavior techniques
• Observes, records and reports members served social and psychiatric behavior
• Reviews “Facility Quality Indicator Profile” reports and makes recommendations to the QI Committee
• Must assist with restraint of members served in the event of assaultive behavior and pass assault crisis/crisis prevention training
Duties and responsibilities may be added, deleted and/or changed at the discretion of management.

QUALIFICATIONS
Required:
• One (1) year of experience in a skilled nursing facility
• Licensure as a Registered Nurse by the State of California and graduation from an accredited school of nursing
• Must be at least 18 years of age
• Must be CPR, Crisis Prevention Institute (CPI), and First Aid certified on date of employment or within 60 days of employment and maintain current certification throughout employment
• All opportunities at Telecare are contingent upon successful completion and receipt of acceptable results of the applicable post-offer physical examination, 2-step PPD test for tuberculosis, acceptable criminal background clearances, excluded party sanctions, and degree or license verification. If the position requires driving, valid driver’s license, a motor vehicle clearance, and proof of auto insurance is required at time of employment and must be maintained throughout employment. Additional regulatory, contractual, or local requirements may apply.

Preferred:
• Inpatient psychiatric/medical experience
• BA/BS degree
• Six (6) months experience with the MDS process; basic Resident Assessment Instrument/Minimum Data Sets

SKILLS
• Ability to work with individuals with multiple disabilities, conduct accurate needs assessment, and coordinate the multidisciplinary Team Treatment Care Plan Process
• Basic computer skills

PHYSICAL DEMANDS
The physical demands here are representative of those that must be met by an employee to successfully perform the essential functions of this job.

The employee is occasionally required to sit, squat, kneel, twist, push, pull and lift and carry items weighing 25 pounds or less as well as to frequently walk, stand, bend, reach and do simple and power grasping. The position requires manual deviation, repetition and dexterity as well as Hazardous Exposure (Blood Borne Pathogens, Hospital Waste, Chemical & Infectious).
