LPN Full-time

JOB PURPOSE:

Conducting initial assessments and reassessments for members/clients for the elderly and disabled waiver program.

KEY RESPONSIBILITIES:

1.   Completes initial and annual assessments in client/member’s home and determines member ability to complete IADLs and ADLs.

2.   Completes modified assessments as needed for transfers, hospital and nursing home

discharges, and increased or added services.

3.   Assures members have services to remain in their community and fulfills the fiduciary obligation to meet members’ needs based on guidelines.

4.   Communicates monthly with administrator to receive re-assessment schedule for SOURCE

and CCSP annual review.

5.   Review medications for appropriateness. Makes note of any new medications, dosages, and discontinued medications.

6.   Documents assessment entries into Harmony system within guidelines established by the

agency and/or regulatory groups. Requests medical and hospital records as needed to assist with determining eligibility.

7.   Participates in team meetings with Medical Director weekly to present all approved new admits, reassessments, hospitalizations, Home Health admits and discharges, and repeat variances. Also, members with potential for discharge, actual discharges, and situations threatening someone’s health or functional status.

8.   Attends and participates in mandatory in-services.

9.   Complies with corporate compliance program.

10. Reports job-related functions/tasks that involve occupational hazards including exposure to blood and body fluids and others as necessary.

11. Follows established safety regulations, to include fire protection & prevention, smoking

regulations, and infection control and prevention.

12. Follow established safety procedures when performing tasks and/or working with equipment.

KNOWLEDGE, SKILLS, ABILITIES:

    Must be computer literate and have ability to learn new programs.

    Must be able to complete tasks in compliance with timelines created by the Department of Community Health (DCH).

    Must be able to communicate with translators for our non-English-speaking clients/members.

    Keeps supervisor informed of daily activities and issues.

    Demonstrates ability to get along with co-workers; maintain effective relationships with management staff and the general public.

    Carries out all duties with a “Commitment to Caring” and in accordance with the company’s mission, vision, and philosophy.

    Follows all company policies and procedures, state and federal laws and regulations and accreditation requirements.

    Reports any violations to the appropriate supervisor and/or compliance department,

    Demonstrates timely and regular attendance.

    Maintains client/member and company information confidentiality at all times, demonstrates compliance with company Code of Conduct and integrity at all times.

    Responsible for assisting with implementation of the waiver program guidelines and client/member goals working with the Medical Director and Home First team.

    Visits clients/members in their residence to provide care or services according to rules and regulations for elderly and disabled waiver program guidelines.

    Able to carry out the duties and responsibilities of this job (with or without reasonable accommodation) without posing specific or current risk of substantial harm to the health and safety of self and others.

    Able to respond to change productively and handle additional projects as assigned, demonstrates flexibility daily to meet needs of the agency patient/members.

    Assesses the health status of assigned clients/members according to the agency policies and procedures and scope of practice guidelines.

    Meets agency productivity requirements and utilizes time and resources effectively.

    Performs other duties as assigned by Administrator.

    Participates in the agency in-service education program.


MINIMUM EDUCATION REQUIRED:

Nursing degree from an accredited college or university, or a graduate of an accredited/approved license practical nurse (LPN) program.

MINIMUM EXPERIENCE REQUIRED:

Minimum of two (2) years practical nursing experience is required. Experience working with elders and disabled populations required.

MINIMUM LICENSURE/CERTIFICATION REQUIRED BY LAW:

Current and unrestricted Licensed Practical Nurse (LPN) licensure in state of practice.

ADDITIONAL QUALIFICATIONS (Preferred qualifications):

Three (3) years practical nursing experience is required. Previous governmental waiver program experience preferred, or any equivalent combination of experience and training which provides the required knowledge, skills, and abilities.
