Minimum Qualifications
Education and Experience:
• A bachelor's degree which includes a practicum encompassing a substantial number of activities with the target population or which features a major or concentration (concentration = a minimum of 24 credits in the noted disciplines) in: social work, psychology, nursing, rehabilitation, education, occupational therapy, physical therapy, recreation/recreation therapy, counseling, community mental health, child and family studies, sociology, speech and hearing plus one year of case management or other relevant human services experience. A bachelor's degree in any field plus two years of case management or relevant human services experience.
OR
• An associate's degree in a health or human services field (see above fields) and three years of case management experience
OR
• A high school diploma/GED and three years of experience in providing direct services to individuals with mental disabilities or to homeless individuals.
Certifications/Licenses:
- CPR and First Aid certifications (must obtain and maintain).
- Certification as an Opioid Overdose Responder (NYSOOPP).
Work Eligibility: Must be authorized to work in the U.S.

About the Role
The Case Manager organizes and implements service planning, conducts assessments, coordinates linkages, and provides direct support to residents in shelter programs.
Primary Responsibilities
• Participate in resident orientation.
• Complete assessments and maintain service plans using integrated healthcare approaches.
• Provide counseling, daily living instruction, and crisis intervention.
• Transport residents to appointments and support income optimization.
• Facilitate recreational and vocational linkages.
• Conduct inspections, document progress, and complete incident reports.
• Participate in quality improvement and maintain accurate records.
• Attend required meetings, trainings, and supervisions.
• Execute emergency plans including CPR/First Aid.
• Perform additional job-related duties as assigned.
