Summary
  • This position assesses, plans, implements, evaluates and supervises individual patient care according to standards. Accepts responsibility for direction of co-workers in the implementation of plans of care. May be required to work in other areas according to skill of staff and acuity of patients. Provides exceptional service to all patients and customers.
  • The RN Inpatient works under the supervision of a Director and is employed by Mosaic Health System.
Duties
  • Completes assessment/reassessment of patient and documents in medical record.
  • Analyzes the information collected to determine patient’s needs and expected outcomes.
  • Develops and implements plan of care based upon the assessment, diagnosis, and expected outcomes of patient.
  • Ensures a discharge plan is a part of the individualized plan of care as appropriate to the population served.
  • Engages in patient/family/staff teaching to achieve the desired patient outcomes.
  • Performs other duties as assigned.
Qualifications
  • All required education is a minimum requirement. Higher levels of education are acceptable. Graduate of school of nursing required. BSN Preferred.
  • RN  - Registered Nurse - State Licensure And/Or Compact State Licensure. State Licensure And/Or Compact State Licensure in state, depending upon designated work location within 90 days of hire is required.  BLS certification required within 90 days from start date. ACLS  - Advanced Cardiac Life Support IF working in an inpatient area in which ACLS is required within 90 days of start date.
  • Prior clinic or hospital experience is preferred.

Duties Completes an initial assessment of patient and family to determine home care needs. Provides a complete physical assessment and history of current and previous illnesses. Documents all care timely. Regularly re-evaluates patient nursing needs. Reports changes in patient condition to physician. Obtain/receives physician’s orders as required. Develops and imitates the plan of care and makes necessary revisions as patients’ status and needs change. The plan of care is based upon patient assessment and physicians’ orders. Uses health assessment data and OASIS/HIS data to determine nursing diagnosis. Develops a care plan, which establishes goals based on nursing diagnosis and incorporates therapeutic, compassionate, preventive, and rehabilitative nursing actions. Includes the patient and the family in the planning process. Initiates appropriate preventative and rehabilitative nursing procedures. Administers medications and treatments in prescribed by the physician. Qualifications Nursing Diploma or Degree from an accredited school of nursing is required, BSN preferred. Current certification or licenses as required by state law within the practicing state. BLS certification required within 90 days from start date. Valid and in state drivers license is required upon hire. Minimum of one-year acute care or public health experience, Home Care experience preferred.
