Phoenix Hospice RN Case Managers provide compassionate nursing care to clients and their families dealing with a terminal diagnosis. Will work with the interdisciplinary team to develop and implement a plan of care that is patient/family centered, with goals of care discussions and guidance with healthcare decisions to give quality of life.

Responsibilities

Provide education to patients/family members regarding the process, plan of care, and offers guidance with healthcare decisions.
Consult with the attending physician and Hospice Medical Director concerning changes in patients condition requiring order changes and/or changes to plan of care.
Coordinate hospice care by working with other team members as well as with the patients/family members to ensure needs are met.
Conducts initial as well as on-going comprehensive assessments.
Preform evaluations for hospice care along with informational visits.

Requirements

Missouri or Compact RN License
Hospice or Long-Term Nursing experience.
Valid driver’s license and auto liability insurance.

Responsibilities and Purpose Provide bedside care. Prepare and administer injections and/or enemas. Collect samples and monitor catheters. Provide wound care. Obtain and administer prescribed medications and treatment in accordance with approved nursing techniques. Ensure the residents’ chart is current. Observe residents and record significant conditions and reactions. Notify appropriate parties of residents’ change in condition and reaction to drugs, treatments, and significant incidents. Take vital signs to detect deviations from normal and assess the condition of residents. Keep residents’ family and physician informed of any changes in resident behavior, medications, or conditions of health. Modify residents’ care plan, care directives, Activities of Daily Living (ADL) and Nurse Aide flow sheets in conjunction with residents’ changes in condition. Desired Experience and Education Current and unencumbered Oklahoma Registered Nurse License (RN) · · Must have a current BLS CPR card One (1) or more years of experience in a Skilled Nursing Facility or Long Term Care environment preferred.
