About Us – Case Management
The Case Management team initiates discharge planning with every patient upon admission and provides assistance with aftercare, medical equipment, care coordination, and other discharge needs.
Case managers are assigned to individual units but also may follow patients with specific discharge needs to provide targeted, patient-centered care.
The Case Management team collaborates with physicians, nurses, ancillary staff, patients, and families to coordinate excellent care.
Case Managers are critical members of the multidisciplinary team and maintain consistent communication with care providers. The Case Management team strives to achieve timely discharge of patients with all needed aftercare arranged.
What You’ll Do
Responsible for assessing patients’ medical and psychosocial needs upon admission and coordinating all aspects of discharge planning. Facilitates post‑acute care services, including DME, home health, and referrals to SNF, IRF, and LTACH facilities. Collaborates daily with the care team, patients, and families to ensure timely, appropriate discharge plans. Interpret clinical data to guide care decisions, assess medical necessity, and meet payer requirements. Provides education on disease management, medications, and available community resources to support successful transitions of care.
Requirements
Current Missouri Registered Nurse license or current Registered Nurse license from a compact state. If a compact license is held, it must be in the nurse’s state of residence.
AHA BLS for Healthcare Providers within 30 days.
Clinic Competency Assessment annually by transcript due date.
