Job Summary: Aspire Senior Living Webb City is seeking a detail-oriented LTC MDS Coordinator to manage and ensure the accuracy and compliance of the Minimum Data Set (MDS) assessments for residents. This role involves coordinating assessments, care plans, and ensuring compliance with federal and state regulations in a long-term care (LTC) setting.

Key Responsibilities:

  • Complete and oversee MDS assessments and documentation
  • Ensure timely submissions to CMS
  • Collaborate with interdisciplinary teams for care planning
  • Monitor resident care plans for accuracy and effectiveness
  • Maintain compliance with state and federal regulations

Qualifications:

  • RN/LPN licensure in Missouri 
  • Experience in long-term care and MDS coordination
  • Strong organizational and communication skills
  • Proficiency in MDS software and CMS regulations
