Sandra Eskenazi Mental Health Center, Indiana's first community mental health center, provides comprehensive care for emotional and behavioral problems, including severe mental illness and substance abuse. The Sandra Eskenazi Mental Health Center offers both inpatient and outpatient services, including several outreach centers as well as clinic- and community-based services.

Job Role Summary

The Sandra Eskenazi Mental Health Center Staff Nurse performs nursing services as a health team member in Outreach clinics. This position provides health education and patient counseling to control and prevent disease and promote optimum health and wellbeing of clients. This position responds to health needs of patients when seen in the clinic by identifying and assessing health care requirement of problems expressed.

Essential Functions and Responsibilities

  • Performs nursing services as a health team member in Outreach clinics
  • Provides health education and patient counseling to control and prevent disease and promote optimum health and wellbeing of clients
  • Responds to health needs of patients when seen in the clinic by identifying and assessing health care requirement of problems expressed
  • Assists physicians and other team members in clinics
  • Develops and implements nursing care interventions based upon goals in the Plan of Care
  • Refers clients to appropriate agencies/organizations to promote continuity of care for patients and families based upon nursing assessment responses from the client/family
  • Works collaboratively with physicians and other health professionals to implement treatments, dispense medications, and give medication and injections in accordance with prescribed orders, instructions
  • Works as a member of a health team to counsel clients and their families in health education, medication education and treatment instructions
  • Attends staff meetings regularly and punctually to share information regarding patients, policies/procedures, peer review, and resource material
  • Completes and submits internal nursing audits regularly
  • Interviews patients and records health history, prepares, maintains and uses records for reports for direct client services
  • Alters activities/behaviors to reflect/ensure adequate care appropriate to the age of patients served (i.e. adolescent, geriatric, pediatric, neonatal, adult) as directed in the area/unit age-specific and/or job specific competencies
  • Ensures the timely completion of all documentation per policy
  • Meets productivity goal as set forth by the agency
  • Demonstrates an understanding of The Recovery Model

Job Requirements

  • Minimum of twelve months of clinical experience in Behavioral Health
  • Minimum of 12 months clincal nursing experience as a Registered Nurse
  • Current Indiana License as Registered Nurse
  • Must have valid Indiana Driver’s License
